摘 要:目的 探讨高强度聚焦超声 (HIFU)非侵入性凝固局部心肌组织的能量 -效应关系。方法 采用不同强度 (110 0 0W /cm2 、15 80 0W /cm2 和 2 2 2 0 0W /cm2 )的HIFU ,在不同辐照时间 (1s、3s、5s和 8s)的作用下 ,对 2 0只正常猪离体心脏进行定位损伤 ,观察并测定损伤区的形态及体积 ,损伤区及其与周围正常组织交界处的组织同时送病理学检查。结果 不同剂量下HIFU所致的损伤体积范围分布在 (11.2± 1.9)mm3 ~(2 83 .2± 4.5 )mm3 之间 ,不同处理因素间的损伤体积差异具有显著性意义 (P <0 .0 5 )。损伤形态随剂量增大趋向不规则。组织学观察可见损伤区与周围正常组织分界明显。结论 HIFU可有效地使心肌组织发生凝固性坏死 。Objective To explore the dose effect relation of swine in vitro myocardial tissue exposed to high intensity focused ultrasound (HIFU).Methods The JC therapeutic apparatus was separately used to selectively destroy the different parts in myocardial tissue of 20 fresh normal swine in vitro hearts which just treated by a vacuum pump.In the step,the tissue was exposed to HIFU with the acoustic intensity of 11 000 W/cm 2, 15 800 W/cm 2 and 22 200 W/cm 2 respectively for 1 s,3 s,5 s and 8 s.The autopsy was applied and then the lesions were measured,and pathologic changes were observed with a microscope.Results The lesion volume varied from ( 11.2 ± 1.9 )mm 3 to ( 283.2 ± 4.5 )mm 3 with the change of exposed time and acoustic intensity,and significant difference was found among them (P< 0.05 ).Marked pathologic changes in regional myocardial tissue were observed by microscope when the in vitro myocardial tissue had been exposed to HIFU.Conclusions HIFU technology is effective to obtain in vitro ablation of myocardial tissue.The shape of lesion area appeared irregular with increased HIFU dose.
