急诊左半结肠切除术中结肠灌洗的应用

摘　　要：目的　研究结肠灌洗在急诊左半结肠一期切除吻合术中肠道准确的应用价值。方法　回顾性分析 1990年 1月至 2 0 0 1年 9月收治的 81例急诊左半结肠梗阻病例的外科治疗 ,1995年 5月前 46例术中未行结肠灌洗 (以下称对照组 ) ,1995年 1月后 3 5例施行结肠灌洗 (以下称治疗组 ) ,分析结肠灌洗对手术方式和预后的影响。结果　治疗组较对照组一期手术率提高 (92 .14 %vs.0 .78% ,P <0 .0 1) ,一期手术吻合口漏发生率减少 (0 %vs .86.92 % ,p <0 .0 1) ,伤口感染率降低 (5 .72 %vs .15 .2 2 % ,P <0 .0 5 ) ,住院时间 (天 )缩短 (17± 2vs .3 8± 3 ,P <0 .0 5 ) ,手术死亡减少 (0 %vs.13 .0 4,P <0 .0 1) ,肿瘤患者 3年生存率提高 (5 6.5 3 %vs.41.2 5 % ,P <0 .0 5 ) ,差异有显著性或极显著性意义。Objectives:The purpose of this study was to elucide the value of the application of colon irrigation in acute one-stage resection and anastomosis of left colon acute obstruction.MethodsThe operation of 81 patients who have acute left colon obstruction since 1990 to 2001 were retrospectively analyzed.46 cases didn't receive colon irrigation in operation before 1995.35 cases received colon irrigation in operation after 1995. Effect of colon irrigation were retrospectively analyzed in this study.Results:The ratio of one-stage operation(92.14%vs.0.78%,P<0.01)and 3 year survival of tumour patient(56.53% vs.41.25%,P<0.05) in the contrasts group was significantly higher than that in the controsts group.The complicative incidence[leakage (0% vs.86.92%,P<0.01),wound infection(5.72%vs.15.22%,P<0.05)],mortality(0%vs.13.04,P<0.01),hospitali zed day(17±2 vs.38±3,P<0.05) after colon irrigation in operation decreased significantly.Conclusion:Colon irrigation in operation can raise the security and curability of acute one-stage resection and anstiomosis on left colon obstruction.
