血液透析患者血清瘦素水平与营养状态的研究

摘　　要：目的　研究维持性血透 (MHD)患者血清瘦素 (Leptin)的水平及与营养状态的关系。方法　采用放射免疫测定 (RIA)方法 ,测定 5 5例MHD患者和 4 0例正常健康人血清瘦素水平 ,同时检测血浆白蛋白、总蛋白、甘油三酯、胆固醇、高密度脂蛋白 (HDL)、低密度脂蛋白 (LDL)及血清肌酐、尿素氮 (BUN)等指标。结果　⑴健康对照组及MHD组男性血清瘦素水平明显低于同组女性 (P <0 0 0 1,P <0 0 1) ,MHD男性和女性患者血清瘦素水平均显著高于同性别健康对照 (P均 <0 0 5 )。⑵MHD男性、女性患者血清瘦素水平均与体重指数 (BMI)呈显著正相关 (P <0 0 1,P <0 0 0 1) ,与血浆白蛋白、总蛋白、甘油三酯、胆固醇、HDL、LDL及血清肌酐、BUN无明显的相关关系。结论　健康女性及MHD女性血清瘦素水平均明显高于同组男性 ;MHD男性、女性血清瘦素水平均高于同性别健康人 ,且与BMI呈正相关而与血浆白蛋白、总蛋白、甘油三酯、胆固醇、HDL、LDL及血清肌酐。Objective To investigate serum leptin levels in maintenance hemodialysis (MHD) patients with chronic renal failure and the relationship between serum leptin levels and nutriture.Methods After hemodialysis,the brachial triceps skin fold was determined in MHD patients,then counted the body mass index(BMI) Serum leptin levels of 55 MHD patients and 40 normal controls were measured by radioimmunossay (RIA) .Simultaneously, other biochemical parameters;BUN, creatinine, serum albumin, triglyceride, cholesterol ,HLD,LDL etc also were mensured.Results ⑴Serum leptin levels in MHD patients were significantly higher than that in normal controls(P<0 05).⑵Serum leptin levels in MHD group and normal control group correlated positively with their BMI,total systemic flow(TSF) in male and famale(γ=0 4532,γ=0 6569,γ=0 4622,γ=0 5892,P<0 01,P<0 001,P<0 01,P<0 001 respectively;). Serum leptin levels in MHD patients were no significantly correlated with their BUN, creatinine, serum albumin , triglyceride, cholesterol,HLD,LDL.Conclusions Serum leptin levels were increased in MHD patients . Serum leptin levels were significantly correlated with their BMI,but that no significantly correlated with their BUN, creatinine, serum albumin , triglyceride, cholesterol,HLD,LDL.
