摘 要:目的 探讨狼疮性肾炎 (L N)患者外周血淋巴细胞亚群、活化状态和免疫球蛋白的变化及其相互关系 .方法 采用流式细胞仪和酶联检测仪对 2 1例 L N进行了研究 .结果 1L N患者 CD3+ +CD4 + (0 .2 4± 0 .10 ) ,CD4 5 RA+ (0 .2 9± 0 .0 6 ) ,CD4 5 RO+ (0 .0 2± 0 .0 2 ) ,CD4 + +CD4 5 RA+ (0 .11± 0 .0 6 ) ,CD4 + CD4 5 RO+ (0 .0 5± 0 .0 4 )细胞比例明显减少 ,与对照组 (0 .4 1± 0 .0 9,0 .4 5± 0 .11,0 .0 5± 0 .0 5 ,0 .2 1±0 .0 7,0 .0 8± 0 .0 2 )比较 P<0 .0 5 ,P<0 .0 1;2 L N患者 HL A-DR+ (0 .13± 0 .0 4 ) ,CD19+ +HL A- DR+ (0 .0 8± 0 .0 3) ,CD19+ +CD4 3+ (0 .0 3± 0 .0 2 ) ,CD19+ +CD5 + (0 .0 5± 0 .0 2 )细胞比例明显增高 ,与对照组 (0 .0 6± 0 .0 4 ,0 .0 4± 0 .0 4 ,0 .0 1± 0 .0 0 4 ,0 .0 1± 0 .0 1)比较 P<0 .0 5 ,P<0 .0 1;3L N患者 Ig E水平明显升高 ,与对照组 (81± 4 0 ) k U· L- 1 相比 ,NS组 (46 7± 2 99) k U· L- 1 P<0 .0 1,非 NS组 (2 2 4± 15 1) k U·L- 1 P<0 .0 5 ,两组比较 P<0 .0 5 ;Ig M,NS组 (3.7± 2 .0 )g· L- 1 ,非 NS组 (3.6± 2 .9) g· L- 1 ;Ig A,NS组 (4.7±1.1) g· L- 1 ,非 NS组 (4.1± 2 .0 ) g· L- 1 水平也升高AIM To investigate the distribution of activated state of lymphocyte in peripheral blood and levels of serum immunoglobulins in patients with lupus nephritis. METHODS The immunophenotype of peripheral blood lymphocytes was analysed by flowcytometry with double labeling anti CD3, anti CD4, anti CD5, anti CD19, anti CD43, anti CD45RA, anti CD45RO, HLA DR mAbs, while the levels of serum IgE, IgG, IgA, and IgM were detected by ELISA. The experimental groups were Group A (9 cases with nephritic syndrome), Group B (12 cases without nephritic syndrome) and control group (30 cases of normal volunteers). RESULTS ① The percentages of CD3 ++CD4 + (0.24±0.10), CD45RA + (0.29±0.06), CD45RO + (0.02±0.02), CD4 ++CD45RA + (0.11±0.06), CD4 ++CD45RO + (0.05±0.04) cells in LN patients were lower than those in normal volunteers (0.41±0.09, 0.45±0.11, 0.05±0.05, 0.21±0.07, 0.08±0.02) ( P <0.05 or P <0.01); ②The percentages of HLA DR + (0.13±0.04), CD19 ++HLA DR + (0.08±0.03), CD19 ++CD43 + (0.03±0.02), CD19 ++CD5 + (0.05±0.02) cells in LN patients were higher than those in normal volunteers (0.06±0.04, 0.04±0.04, 0.01±0.004, 0.01±0.01) ( P <0.05, P <0.01); ③ The serum IgE level in Group A (467±299) kU·L -1 and Group B (224±151) kU·L -1 of LN patients was higher than that in normal volunteers (81±40) kU·L -1 ( P A<0.01, P B< 0.05 ); Group A was lower than Group B, ( P <0.05); the serum IgM, IgA levels in Group A (3.7±2.0, 4.7±1.1) g· L -1 and Group B (3.5±1.9, 4.1±2.0) g·L -1 of LN patients were higher than those in normal volunteers (1.8±0.9, 2.7±0.7) g·L -1 ( P <0.05); the serum IgG level (6.0±4.8) g· L -1 in Group A of LN patients was lower than that in normal volunteers (18.2±5.6) g·L -1 ( P <0.01). In contrast serum IgG level in Group B (30.5±17.5) g·L -1 was higher than that in the normal volunteers ( P <0.01); Group A was lower than Group B ( P <0.01). CONCLUSION Th
