振源胶囊治疗冠心病心肌缺血临床观察

摘　　要：目的观察振源胶囊治疗冠心病心肌缺血的疗效。方法将138例患者随机分为治疗组108例、对照组30例,服药后4周观察临床疗效;治疗前后观察:(1)心率与血压乘积(RPP),(2)心电图ST段压低总导联数(NST)、ST段压低总和(∑ST),(3)彩色多普勒超声检查之心脏每搏出量(SV)、每分输出量(CO)和射血指数(EF),(4)血液流变学指标中全血粘度、血浆粘度、纤维蛋白原,(5)电镜下血小板形态变化。结果治疗后治疗组NST、RPP、∑ST明显降低(P<0.01),SV、CO、EF明显升高(P<0.01),全血粘度、血浆粘度降低(P<0.05),血小板形态由聚集型向树枝型转化明显增多(P<0.01),其次为向圆形转化(P<0.05);对照组NST、∑ST有所降低(P<0.05),SV、CO、EF升高(P<0.05),全血粘度、血浆粘度无明显变化(P>0.05),电镜下血小板形态各型基本无变化(P>0.05)。结论振源胶囊治疗冠心病在改善心肌缺血、增强心功能和降低心肌耗氧量以及改善血流粘滞、血小板聚集等方面确有较好疗效。Objective:To observe therapeutic effects of Zhengyuan capsule in treatment of myocardial ischemia of coronary beart disease.Methods:137 patients with coronary heart disease were randomly divided into treated group (n=108) and controlled group (n=30).Observed the effect after 4 weeks treatment.And (1)Products of heart rate and blood pressur(RPP),(2)the total leads of depression of ST segment (NST)sum of depression of ST segment (∑ST),stroke volume(SV) by electrocardiography and 2-dimensional echocardiography,(3)cardiac output(CO),ejection fraction(EF),(4)blood viscosity,plasma viscosity ,fibrinogen,(5)changes of platelet-shape by electron microscope were observed respectively before and after treatment.Results:In treated group,NST,RPP,∑ST and ηb,ηp decreased,and SV,CO,EF increased cleary.In addition,it was increased that gathering-shape of platet transformed to branch-shape,to disc-shape.In control group NST,∑ST yielded and SV,CO,EF(P<0.05) but for no clear changes of plasma viscosity,fibrinogen and platelet basical shape. Conclusion:Zhenyuan capsule had a good effect on treatment of coronary heart disease in ameliorating myocardial ischemia,improving heart function,reducing oxygen consumption,blood viscosity and platelet shape.
