摘 要:目的 研究左旋精氨酸 (L ARG)对未成熟心肌缺血再灌注损伤的保护作用。方法 实验采用幼兔离体心脏灌注模型 ,实验分为 :①对照组 :以K H液 (Krebs Henseleitsolution)再灌注 ;②L ARG组 ;K H液加入L ARG( 1mmol/L) ;③N 亚硝基左旋精氨酸 (L NNA)组 :加入L ARG( 1mmol/L)和L NNA( 2mmol/L)。各组均以Thomas2号液为心停搏液 ,15~ 2 0℃缺血 90min ,再灌注 30min。测定缺血前 ,再灌注中心功能指标和冠脉流出液中一氧化氮、内皮素含量以及缺血前、再灌注后心肌组织脂质过氧化产物丙二醇含量。结果 ①对照组和L NNA组再灌注后一氧化氮含量下降 (P <0 0 5 ) ,L ARG组无下降 ,②L ARG组再灌注后心功能恢复优于对照组和L NNA组 (P <0 0 5 )。③L ARG组再灌注后心肌组织丙二醛含量和冠脉流出液中内皮素含量低于对照组和L NNA组 (P <0 0 5 )。结论 未成熟兔心肌缺血再灌注后一氧化氮合成释放减少 ,再灌注早期补充左旋精氨酸增加一氧化氮生成量。左旋精氨酸对未成熟心肌缺血再灌注损伤具有良好的保护作用。Objective To explore the effects of L-arginine (L-ARG) on isolated beating immature rabbit hearts with ischemia/reperfusion injury. Methods Thirty six rabbits at age of 16-20 d were divided into 3 groups, Control group, infused with Krebs-Henseleit solution (KHS); L-ARG group, receiving the reperfusion of KHS+L-ARG (1 mmol/L); and NG-L-nitro-arginine (L-NNA) group, given the infusion of KHS+L-ARG (1 mmol/L)+L-NNA(2 mmol/L). Animals of each group were subjected to 90 min of global ischemia arrested by 4 ℃ Thomas-Ⅱ cardioplegic solution and followed by 30 min of reperfusion. L-ARG and L-NNA were infused in the first 15 min postischemia. Parameters on cardiac function, nitric oxide (NO) level and endotheliotoxin (ET) content in fluid of coronary were measured before ischemia and during reperfusion. Content of malodialdehyde (MDA) was investigated before ischemia and after reperfusion. Results L-ARG group achieved better recovery of cardiac function than the other 2 groups, sound content of NO, control group, but poorer recovery of MDA and ET contents. These effects were reversed to equal control values by adding L-NNA. Conclusion ①The production of nitric oxide is decreased during immature myocardium after ischemia/reperfusion. ②L-ARG can protect immature myocardium from ischemica /reperfusion injury.
