双功能彩色多普勒对颈内动脉狭窄的诊断

摘　　要：目的 :探讨双功能彩色多普勒 (DCDU)对颈内动脉粥样硬化性狭窄的诊断和应用价值。方法 :分析4 5例颈内动脉狭窄的 DCDU诊断及其血流动力学资料 ,主要是颈内动脉狭窄处的收缩期峰值流速 (SPV)及舒张末期峰值流速 (EDV)。结果 :4 5例中有 2 3例患者检出硬化斑块 34个 ,73侧颈内动脉起始段有不同程度的狭窄 ,左侧颈内动脉狭窄 4 5侧 ,右侧狭窄 2 8侧。轻度狭窄 2 1例 ,SPV为 (5 1.2 3± 10 .33) cm/ s,EDV为 (16 .86± 5 .31) cm/s;中度狭窄 14例 ,SPV为 (111.16± 13.78) cm/ s,EDV为 (2 8.13± 6 .2 3) cm/ s;重度狭窄 10例 ,SPV为 (2 36 .5 6± 2 1.4 5 ) cm / s,EDV为 (6 3.6 5± 6 .96 ) cm/ s。结论 :利用 DCDU可尽早发现颈内动脉的病变 ,以便早期进行颈内动脉内膜剥脱术的治疗 。Objective:To evaluate the value of DCDU in the diagnosis of internal carotid arteriostenosis(ICAS).Methods:Retrospective analysis was performed on 45 patients with ICAS by DCDU and their hemodynamics changes,particularly on SPV and EDV at the site of stenosis.Results:In all the patients 23 had atheromatous plaques.There were 73 internal carotid arteries which were stenotic at the initial parts,45 at the left side and 28 at the right side.The means and standard variances of SPV and EDV of patients with mild,moderate and severe ICAS were respectively 51 32±10 33cm/s and 16 86±5 31cm/s;111 16±13 78cm/s and 28 13±6 23cm/s;236 56±21 45cm/s and 63 65±6 96cm/s,respectively.Conclusions:Using DCDU we can discover lesions of the ICAS.Thus endarterectomy can be early operated and the incidence of cerebral infraction can be greatly decreased.
