继发性甲状旁腺功能亢进的超声引导下介入性治疗

摘　　要：目的 :探讨超声引导下介入性治疗继发性甲状旁腺功能亢进 (甲旁亢 )的疗效与价值。方法 :观察、比较 33例患者经超声引导无水酒精硬化治疗前后血清 PTH水平的变化及甲状旁腺超声图像变化 ,评价其治疗效果。结果 :33例患者经治疗后 ,其血清 PTH全部明显下降。缓解率 15 .2 % (5 / 33) ,显效率 5 4 .5 % (18/ 33) ,满意率 30 .3%(10 / 33)。结论 :超声引导下经皮注入无水酒精治疗继发甲旁亢是一项安全、有效的治疗手段 ,值得推广使用。腺体内血流信号的变化是观察疗效的敏感指标。对难治性继发甲旁亢患者应采用综合性治疗手段 ,以巩固。Objective:To investigate the effective value of interventional therapy of secondary hype rparathyroidism by ultrasonic guide.Methods:The sonography,PTH before and after intervene therapy by ultrasonic guide of thirty three patiends were analysed and compared.Results:After therapy,PTH of 33 patiends all reduced.Rate of remission was 15 2%,rate of remarkable effect was 54 5%,rate of satisfaction was 30 3%.Conclusions:Ultrasound guided percutaneous ethanol injection therapy of the parathyroid in chronic dialysis patients with secondary hyperparathyroidismis is an effctive and safe therapy.Blood signals in parathyroid is a sensitive index to judge curative effect.That would rise and consolidate curative effect to take combined therapy in intractable patiends.
