摘 要:目的 观察糖代谢异常患者血清瘦素含量。方法 将 131例糖代谢异常患者按病情分为糖耐量减退组 (减退组 )和 2型糖尿病组 (糖尿病组 ) ,并以 41名健康人作为对照组 ,测定受检者血清瘦素含量及其他有关指标 ,在瘦素及其相关指标之间做多因素回归分析。结果 减退组和糖尿病组空腹瘦素 (F Lp)、空腹胰岛素 (FINS)、体重指数 (BMI)、腰 /臀比值 (WHR)均高于对照组(P <0 .0 5~ 0 .0 0 1) ,但减退组和糖尿病组比较 ,差异无显著性 (P >0 .0 5 )。减退组空腹血糖(FBG)、糖化血红蛋白 (HbA1C)均高于对照组 ,低于糖尿病组 (P <0 .0 5~ 0 .0 0 1) ;糖代谢异常患者血糖、病程与瘦素含量不相关 ,BMI、WHR和FINS水平与瘦素含量独立相关。结论 糖代谢异常患者血糖与血清瘦素含量无相关性 ,超重肥胖、脂肪分布异常和高胰岛素血症是影响血清瘦素含量的主要因素。Objective To observe the serum leptin level in patients with abnormal glucose metabolism. Methods 131 patients with abnormal glucose metabolism were divided,according to their condition of illness into IGT group(group I) and type 2 diabetes mellitus group (group D),and 41 healthy subjects were employed as control (group C);The leptin concentration in serum and other related parameters of these examinees were determined, and multiple factor regression analysis was performed between the leptin and other related parameters. Results The fasting leptin (F LP), fasting insulin(F INS), body mass index(BMI)and waist/hip ratio (WHR)of groups I and D were higher than those in group C ( P <0.05~0.001), but there was no significant difference between group I and group D( P >0.05); the fasting blood glucose(FBG)and glycosilated hemoglobin(HBA1C)of group I were higher than those in group C but lower than those in group D( P <0.05~0.001); The blood glucose level and course of disease of patients with abnormal glucose metabolism were not related to the leptin concentration, and the BMI?WHR and F INS were independently correlated with leptin concentration. Conclusion The blood glucose level of patients with abnormal glucose metabolism were not related to the leptin concentration, overweight and obesity, abnormal fat distribution and hyperinsulinemia are main factors that influence the serum leptin concentration of patients with abnormal glucose metabolism.
