系统性红斑狼疮患者红细胞免疫功能与IL-6、IL-8关系的研究

摘　　要：目的 :研究系统性红斑狼疮 (SLE)患者红细胞免疫功能及血浆IL - 6、IL - 8的改变并探讨其关系。方法 :81例SLE患者分别予以狼疮活动积分 ,分为活动期和缓解期。随机选取 30例健康人作对照。选用C3bRR及ICR作为红细胞免疫功能的指标 ,同时检测血浆中IL - 6、IL - 8的含量。结果 :SLE患者C3bRR率与健康人比较显著降低 (P <0 .0 1 ) ,SLE患者血浆IL - 6、IL - 8水平和ICR率与健康人比较显著升高 (P <0 .0 1 )。C3bRR率与血浆IL - 6、IL - 8水平呈显著负相关 (P <0 .0 1 )。SLE患者活动期C3bRR率较缓解期显著降低 ,而活动期ICR率、血浆IL - 6、IL - 8水平较缓解期显著升高 (P <0 .0 1 )。结论 :SLE患者红细胞免疫功能的改变可以作为判断狼疮活动的指标之一 ;红细胞可以通过其表面的免疫物质调节血浆中炎症介质的浓度从而缓冲炎症的发展 ;SLE患者血浆IL - 6、IL -Objective:To determine the variation of red cell immune function and plasma levels of IL-6 and IL-8 in patients with SLE.Methods:Eighty_one patients with SLE were scored with Lupus Activity Index,cases of ≥3 points were regarded as active stage,and cases of <3 points as stable stage.The normal control group consisted of 30 healthy persons.We selected C 3b RR and ICR as representatives of red cell immune function and tested the plasma levels of IL-6 and IL-8.Results:The C 3b RR rate in patients with SLE were significantly lower than those in normal controls ( P <0.01).The plasma levels of IL-6 and IL-8 and the ICR rate in patients with SLE were significantly higher than those in normal controls( P <0.01).The C 3b RR rate had a significantly negative correlation with plasma levels of IL-6 and IL-8. The C 3b RR rate of active stage were significantly lower than that of stable stage in patients with SLE, however the ICR rate and plasma levels of IL-6 and IL-8 of active stage were significantly higher than those of stable stage in patients with SLE ( P <0.01).Conclusions:The variation of red cell immune function in patients with SLE can be used as one of the indexes of lupus activity controls.Erythrocytes can modulate plasma inflamma_tory cytokines concentration trough their surface substance and buffer the development of inflammation.Plasma levels of IL-6 and IL-8 in patients with SLE are helpful to monitor lupus activity.
