椎管内、外性腰腿痛患者软组织病损评价及实施物理治疗的效果

摘　　要：目的观察腰腿痛患者软组织病损的评价情况和物理治疗的效果。方法应用作者编制的腰腿痛评价表作为评价方法,对比椎管内性腰腿痛(30例)和椎管外腰腿痛(39例)患者各组软组织评价得分和压痛点阳性率的差异。给予两组患者的软组织病损以超短波。调制中频电流和按摩治疗。结果所有患者(69例)均有不同程度的软组织病损。但两组间仅骶棘肌压痛的平均得分有非常显著差异(P<0.01)。其余软组织的平均得分对比无显著差异(P>0.05)。两组各软组织压痛点阳性率对比无显著差异(P>0.05)。两组物理治疗前和治疗后腰腿痛评价表各分项得分和总分对比均有显著差异(P<0.05～0.01)。结论椎管内性腰腿痛或椎管外性腰腿痛患者均存在一定程度的软组织病损,影响患者的症状和功能。Objective To observe the evaluating outcomes a nd physical therapy effects of soft t issue disease in low back pain(LBP)pa-tients.Methods Using the evaluaion scale for low bac k pain(ESLBP)as assessing method,the difference s of the scores and positive rates of soft tissue tenderness points betwe en intraspinal disease LBP group(30cases)and extraspinal disease LBP group(39cases)were compared.The soft tissue diseases of patients in the two groups were treated by ultr ashort wave,modulated medium frequ ency electrotherapy and massage for 1～2course(15days a course),one times a day.The patients of intr aspinal group were also treated by lu mbar traction.Results All patients(69cases)had soft tissue tenderness points.B ut only the scores of musculi sacrosp inalis between two groups had great difference(P<0.01).The scores of other soft tissues between two groups had no obvious difference(P>0.05).The positive rates of tenderness points be-tween two groups had no obvious diffe rence(P>0.05).The differences of total scores and all subentry scores of ESLBP between before and after physical treatment in the two g roups were all significant (P<0.05～0.01).Conclusion No matter intraspinal or extraspinal disease LBP patients,soft tissue diseases a re existent in.Physical therapy has good effects in the treatment of soft tissue disease and in the improvemen t of the symptom and function of the LBP patients.
