复杂性胫骨平台骨折的手术疗效分析

摘　　要：目的 :探讨手术治疗复杂性胫骨平台骨折 ,即 Schatzker 、 和 型骨折的疗效。方法 :收集 1997年～ 2 0 0 0年在我院行切开复位内固定治疗的复杂胫骨平台骨折 3 6例进行回顾性分析 ,全部病例术前进行详细的手术设计 ,螺旋 CT平扫及三维图像重建 ,制订手术方案 ,术中 T或 L型钢板固定。结果 :随访 10个月～ 5年 ,平均 2 6个月 ,按 Mer-chant评分标准 ,优良率 83 .3 %。结论 :(一 )切开复位支撑钢板固定治疗复杂性胫骨平台骨折疗效满意 ;(二 )术前结合螺旋 CT检查制定详细手术计划 ,术中力争解剖复位 ,足量植骨 ,坚强固定 ,术后完备的康复计划是手术成功的关键。Objective:To analyze the effect of operation on Complex Fractures of the Tibial plateau.Methods:Thirty-six cases complex tibial plateau fractures with open reduction and internal fixation had been reviewed.All cases had been designed carefully before operation,with CT scan and recoustruction of three-dimensions,with T or L-shaped plate fixation.Results:The follow-up was 10 to 60 months.Good result was 83.3% in all cases,according to the Merchant′s criteria.Conclusion:(1) It is effective to the complex tibial plateu fractures with open reduction and internal fixation;(2) In order to get good result,It is important to make up a careful design before operation,regulate the rule of operation and the detail plan of exercise of postopreation.
