血液透析滤过对慢性肾衰患者LPS、IL-1β、TNFα的影响

摘　　要：目的 :研究血液透析滤过 (HDF)对外周血细菌内毒素 (LPS)、白细胞介素 - 1β(IL - 1β)、肿瘤坏死因子(TNFα)的清除作用。方法 :14例慢性肾功能衰竭 (CRF)患者 ,随机选择 7例HDF和血液透析 (HD)交替 ,另 7例设为对照组 ,只予HD。酶联免疫法检测首次治疗前、后及治疗三个月后血LPS、IL - 1β、TNFα浓度。 结果 :(HDF +HD)组首次治疗后血清LPS、IL - 1β较治疗前有所下降 ,治疗后与HD组比较LPS、IL - 1β水平下降 ,治疗前、后及组间比较均有显著差异。治疗三个月后 (HD +HDF)组血清LPS、IL - 1β、TNFα浓度较HD组降低 ,P <0 .0 5。结论 :HDF可部分清除LPS、IL - 1β ,且可使CRF病人血清TNFα水平下降。Objective: To study the effects of hemodiafiltration (HDF) on clearing Lipopolysaccharide(LPS)?interleukin-1β(IL-1β)? tumor necrosis factor-α(TNFα) in patients with chronic renal failure(CRF). Methods:14 CRF patients were randomly divided into two groups. HDF group (n=7)and control group(n=7). Control group was treated with hemodialysis(HD), while HDF group was alternate treated with HDF and HD. ELISA method was used to detect the plasma levels of LPS ?IL-1β? TNFα in the three periods:before treated, after treated and three months after treated.Results: In both HDF group and control group , the levels of LPS and IL-1βdecreased after treated and HDF group decreased more than control group. There is significant difference. Three months after treated,the serum levels of LPS ?IL-1β?TNFαin the HDF group are all lower than that in the control group(P<0.05).Conclusions: HDF can not only partly clear LPS and IL-1β, but aslo decrease the TNFα level in CRF patients.
