骨骺突出指数对儿童股骨头缺血性坏死包容手术选择的价值

摘　　要：目的　为探讨骨骺突出指数对儿童股骨头缺血性坏死包容手术选择应用的实际价值。方法　对Salter和Chiari手术方法治疗的 4 5例CatterallⅢ、Ⅳ (SalterB)病例进行分析。结果　 4 5例患儿正位X线片下测得其骨骺突出指数在 8%～ 2 0 %之间2 4例 ,2 2 %～ 5 3%之间 2 1例。术后结果以Mose法评定 ,Salter组 2 7例 ,优 12例 ,良 9例 ,差 6例 ;Chiari组 18例 ,其中优占 9例 ,良 6例 ,差 3例。Salter组差者 6例中骨骺突出指数均大于 2 0 %。结论　骨骺突出指数在骨盆包容手术方法选择及预后判定有重要参考价值。应把小于 2 0 %的骨骺突出指数纳入对Salter手术选择的重要参数指标 ;骨骺突出指数大于 2 0Objective To evaluate the value of epiphyseal extrusion index to choice of the operations of covering for Legg-Calve-Perthes disease.Methods Forty five patients treated with Salter or Chiari operation were analyzed.Results Of the 27 cases treated with Salter operation,12 cases were excellent,9 cases were good and 6 cases were bad by Mose's evaluation method.The epiphyseal extrusion indexs of 6 bad cases were >20% and those of the other 21 cases were <20%.Conclusion Cases of <20% epiphyseal extrusion indexs should be treated with Chiari operation.
