济南某部乙型肝炎发病率调查与预防对策研究

摘　　要：目的　了解部队急性病毒性肝炎与乙型肝炎实际发病率及流行病学特征 ,为部队肝炎防治提供科学依据。方法　以整群抽样方式抽取观察对象 ,收集相关资料和血清标本 ,用 EL ISA检测肝炎感染指标和血清学分型。结果　 1993～ 2 0 0 0年某部队发生肝炎 130例 ,发病率为 0 .81‰ ,各年度发病率在 0 .34‰～ 1.0 1‰ ;其中乙型肝炎 6 6例 ,发病率为 0 .4 5‰ ,各年度发病率在 0 .14‰～ 0 .6 7‰ ,并占肝炎发病数的 5 5 .93%。 1993～ 2 0 0 0年总漏报率为 9.2 3% ,各年度漏报率在 0 %～2 3.81%。干部肝炎发病率 (1.6 2‰ )和乙型肝炎发病率 (1.0 2‰ )均高于战士 (0 .90‰、0 .5 0‰ ) ,新兵检疫仍有 1.6 % HBs Ag阳性。结论　某部队每年肝炎的实际发病率高于报告发病率 ,部队肝炎防治应以乙型肝炎为重点 。Objective In order to provide a scientific data for preventing hepatitis in military troops. Methods Cluster sampling was used to collect data and sera. Hepatitis infections and serology types were detected by ELISA. Results There were 130 cases with hepatitis from 1993 to 2000, and the incidence rate was 0.81‰, and the annual incidence rate changed from 0.34‰ to 1.01‰. There were 66 cases with hepatitis B, the incidence rate was 0.45‰, the annual incidence rate changed from 0.14‰ to 0.67‰ and the constituent ratio of hepatitis B was 55.93%. The average rate of missing report was 9.23% from 1993 to 2000 and the annual rate of missing report changed from 0% to 23.81%. In hepatitis cases, the incidence rate of cadres (1.62‰) was higher than that of soldiers (0.90‰) and the incidence rate of hepatitis B of cadres (1.02‰) was also higher than that of soldiers (0.50‰) and the constituent ratio of positive HBsAg was 1.60% in new soldier quarantine. Conclusions The annual actual incidence rate of hepatitis was higher than that reported and the prevention of hepatitis especially hepatitis B should be strengthened and the quarantine to new soldier should be developed actively.
