摘  要:目的 观察高压氧 (HBO)治疗新生大鼠缺氧缺血性脑损伤后血浆和大脑皮层的神经肽Y1 - 36 (NPY)和降钙素基因相关肽 (CGRP)含量的动态变化。方法  7d龄新生大鼠左侧颈总动脉结扎形成缺血 ,并吸低氧氮混合气形成缺氧 ,随机分为脑缺氧缺血 +常氧高压氮组、脑缺氧缺血 +HBO治疗组、假手术组和正常对照组 ,再按不同时间各分成 4个小组。新生大鼠缺氧缺血性脑损伤后即刻行 HBO治疗 ,用放射免疫分析法测定治疗后不同时间血浆和大脑皮层的神经肽 Y1 - 36 和降钙素基因相关肽免疫活性物质 (ir- NPY、ir- CGRP)含量的变化。结果 缺氧缺血性脑损伤后不同时间 ,血浆 ir- NPY和 ir-CGRP、双侧皮层 ir- NPY、伤侧皮层 ir- CGRP含量不同程度升高 ;HBO治疗 ,可部分抑制血浆 ir- NPY、ir- CGRP的升高 ,左侧皮层 ir- NPY水平的升高被明显抑制 ,右侧皮层 ir- CGRP水平受影响较小。结论 缺氧缺血性脑损伤的发生发展与外周及中枢神经肽 Y1 - 36 和降钙素基因相关肽的过量生成有关。HBO治疗降低外周神经肽 Y1 - 36 和降钙素基因相关肽 ,降低伤侧脑区神经肽 Y1 - 36 的水平 。Objective To investigate the peripheral and central dynamic changes of levels of neuropeptide Y 1 36 and calcitonin gene related peptide after hyperbaric oxygen therapy(HBO) of hypoxic ischemic brain injury(HIBI) in neonatal rats. Methods Seven day old Sprague Dawley rats were randomly divided into four groups:HIBI+normoxic hyperbaric nitrogen group, HIBI+HBO group, sham operated group and normal control group with each group divided into four subgroups according to the time schedule. Neonatal HIBI model was preformed by ligating the left common carotid artery and then by breathing in a 37℃ humidified 8% oxygen and balanced nitrogen at 1.5 L/min for 2.5 h. Changes of levels of both neuropeptide Y 1 36 like and calcitonin gene related peptide like immunoreactives(ir NPY and ir CGRP) in plasma and cortices were tested by radioimmunoassay 0,12,24 and 48 h after HBO treatment. Results Significantly elevated levels of ir NPY and ir CGRP in plasma, ir NPY in left and right cortices, ir CGRP in left cortex were observed at different time intervals after HIBI. High levels of both ir NPY and ir CGRP in plasma were partially attenuated, and high levels of ir NPY in left cortex were significantly reversed with the HBO treatment. Little effect of HBO on levels of ir CGRP of cortex after HIBI was seen. Conclusions Pathophysiological process of HIBI might be related to the peripheral and central excessive secretion of neuropeptide Y 1 36 and calcitonin gene related peptide. One of the pathways of alleviating HIBI by HBO might be the inhibition of these high levels of ir NPY and ir CGRP in plasma and of ir NPY in cortex ipsilateral to the damage.
