充血性心力衰竭患者甲状腺激素水平与心功能的关系

摘　　要：目的　观察 32例充血性心力衰竭 (CHF)患者甲状腺激素的水平及与心功能的关系。方法　CHF患者 32例 ,男 18例 ,女 14例 ,平均年龄 5 8岁 ,于入院后采血进行甲状腺激素水平的测定 ,超声心动图检查测定射血分数 (EF)及心功能。结果　甲状腺激素的水平随心功能不同而有变化 ,心功能Ⅳ级时T3 、T4、FT3 均明显降低 ,与Ⅱ级、Ⅲ级心功能相比有显著性差异 (P <0 0 1或P <0 0 5 ) ,而TSH、FT4、rT3 在心功能Ⅳ级与Ⅱ级、Ⅲ级间无明显差异。相关性分析表明 ,EF与rT3 呈负相关 (r=- 0 74 ) ;EF与TSH呈负相关 (r=- 0 6 8) ;EF与FT3/FT4呈正相关 (r=0 5 9)。结论　CHF时甲状腺激素的代谢发生了明显变化 ,而且心功能参数与甲状腺激素的水平有明显的相关关系。Objective To observe the relationship between level of thyroid hormone and cardiac function in congestive heart failure(CHF).Methods A total of 32 patients with CHF were enrolled in the study, men 18 patients, women 14 patients, means age 58 years old, levels of T 3,T 4, TSH and free T 3,free T 4,rT 3 were measured in all patients. Echocardiogram was examined to evaluate index of cardiac function.Results Level of thyroid hormone have changed in alternative cardiac function,plasma T 3,T 4,FT 3 level were decreased in cardiac function Ⅳ class, there were significant differences compared with cardiac function Ⅱ,Ⅲ class. Correlation analysis was showed that echocardiographic ejection fraction (EF) correlated with rT 3(r=-0 74), EF has negatively correlated with thyroid stimulation hormone(r=-0 68), EF was positively correlated with FT 3/FT 4 ratio (r=0 59) in CHF. Conclusion Thyroid hormone metabolism has a markedly altered in CHF. there was a obiously correlation between cardiac function and thyroid hormone.
