缬沙坦联合美托洛尔治疗充血性心力衰竭疗效观察

摘　　要：目的　观察缬沙坦 (Valsartan)联合美托洛尔(Metoprolol)治疗充血性心力衰竭 (CHF)的临床疗效和安全性。方法　将 78例CHF患者随机分为A组和B组各 39例 ,两组均予常规抗心衰治疗 ,A组加服缬沙坦 80mgqd (1周后增至 80mgbid)。B组加服缬沙坦 80mgqd(1周后增至80mgbid)及美托洛尔 6 2 5mgbid ,若能耐受每 3天增加剂量 1倍 ,剂量为 12 5～ 5 0mg/d ,疗程均为 8周。全组病例治疗前后观察临床疗效、血压 (BP)、心率 (HR)、心胸比、每搏量(SV)、每分输出量 (CO)、心脏指数 (CI)和左室射血分数 (EF)的变化。结果　A组临床显效 2 0 5 %、总有效率 84 6 % ;B组临床显效 30 8%、总有效率 92 3%。组间比较收缩压(SBP)、舒张压 (DBP)、EF及心率B组优于A组 (P <0 0 1) ,CO及心胸比B组优于A组 (P <0 0 5 )。Objective To evaluate the clinical therapeatic efficacy and safety of Valsartan in combination with Metoprolol in patients with congestive heart failure(CHF). Method 78 patients with CHF were divided randomized into group A( n= 39) and group B( n= 39) Each group received Valsartan(at a dose of 80 mg qd at first,and added to 80 mg bid after a week)on the basis of conventional treatment,and besides this group B took Metoprolol (6 25 mg bid or 12 5～50 mg qd),the study continued for 8 weeks,and the clinical efficacy?blood presure(BP)?heart rate(HR)?heart chest ratio was examed and stroke volume(SV)?cardiac output(CO)?cardiac index(CI)and left ventricular ejection fraction (LEVF) were observed Results Effective rate of group A was 20 5% of obvious efficacy and 84 6% of total efficacy;and that of group B was 30 8% of obvious efficacy and 92 3% of total efficacy,contrast to A group,B group have more excellent systolic blood pressure (SBP), diastolic blood pressure(DBP)?EF?HR( P< 0 01),and also CO?heart chest ratio( P <0 05) Conclusion The efficacy of Valsartan combined with Metoprolol is more effective than Valsartan alone in therapy of CHF
