慢性再生障碍性贫血患者免疫功能改变及意义

摘　　要：目的　观察慢性再生障碍性贫血 (CAA)患者红细胞和淋巴细胞免疫功能的改变 ,探讨红细胞和T淋巴细胞免疫功能之间及机体免疫功能和CAA发病之间的关系。方法　采用免疫酶标比色法、免疫比浊法、单克隆抗体免疫萤光法、流式细胞仪检测分析等 ,观察比较CAA患者和对照组的各项免疫指标。结果　与对照组相比较 ,CAA患者IgG、IgA、IgM、CD5 8、RBC C3bRR等显著降低 (P <0 .0 1) ,CD3、CD4、CD8、CD4 /CD8比值等明显异常。结论　CAA患者表现多种免疫功能改变 ;主要在骨髓内分化、增生的红细胞、B淋巴细胞的免疫功能明显低下 ,在骨髓外分化、增生的T淋巴细胞亚群的表达多异常升高。Objective To observe the changes of immune functions of the erythrocytes and lymphocytes in chronic aplastic anemia (CAA) patients, and to investigate the relation between the immune functions of the erythrocytes and lymphocytes, and the association between body immune functions and the episode of aplastic anemia.Methods Immune enzyme marked chromometry ,immunoturbidimetry and monoclonal antibody immunofluorescence method were used to observe and compare each immune data between the CAA group and the control group.Results As compared with the control group, the content of IgG,IgA and IgM in serum,and the expression of CD58 and C3bRR on erythrocytes in CAA group decreased significantly ( P <0.01), the expression of CD3, CD4, CD8 on lymphocytes and the value of CD4/CD8 were obviously abnormal.Conclusion It shows that CAA patients have multiple kinds of changes of immune function. The immune function of erythrocytes and B lymphocytes of which differentiation and proliferation mainly in bone marrow are very poor, while in most of patients, the expression of antigen on T lymphocyte subgroups of which differentiation and proliferation mainly out of bone marrow increase abnormally.
