摘  要:目的 研究HP感染者胃黏膜上皮细胞凋亡与一氧化氮 (NO)之间的关系。方法 分别用切口末端标记法和硝酸还原酶法对HP阳性的十二指肠球部溃疡 (DU)、胃溃疡 (GU)、癌前病变、胃癌以及组织学上正常的HP阴性者各 10例的胃黏膜活检组织进行细胞凋亡和一氧化氮检测。结果 从HP阳性的十二指肠溃疡→胃溃疡→癌前病变→胃癌→HP阴性的正常组织 ,细胞凋亡逐渐减少 ,凋亡指数除在胃溃疡与球部溃疡之间无统计意义外 (P >0 0 5 ) ,其它各组间相互比较差别均有统计学意义 (P <0 0 1)。HP根除以后细胞凋亡显著减少。NO含量在HP阳性的各实验组比HP阴性的正常对照组显著增高 (P <0 0 1) ,但各实验组之间差别均无显著性 (P>0 0 5 )。在HP阳性的十二指肠溃疡、胃溃疡细胞凋亡与NO呈显著正相关 (P <0 0 5 )。而在癌前病变组和胃癌组细胞凋亡与NO无显著相关性 (P >0 0 5 )。结论 ⑴HP感染能诱导胃上皮细胞凋亡 ,这可能是HP致胃癌的机制之一 ;⑵HP感染可能引起胃黏膜NO的升高 ;⑶在HP感染早期 ,大量产生的NO可能有助于胃上皮细胞凋亡。根除HP ,以减少细胞凋亡可能有助于预防胃癌的发生。Objectives To study the effect of Helicobacter pylori( HP) on gastric epithelial apoptosis, and the relationship between apoptosis and nitric oxide(NO)levels in gastric mucosas .Methods Biopsy specimens were taken from patients with HP(+) duodenal ulcer (DU,n=10), gastric ulcer (GU,n=10), gastric precancerosis (n=10), gastric cancer (n=10) and HP( ) normal gastric mucosas. Apoptosis and NO levels in gastric mucosas were detected by nick terminal lable technique and the nitrate reductase methods, respectively. Results Apoptotic Index (AI) were gradually decreased from the duodenal ulcer (17 5%) to gastric ulcer (15 1%), gastric precancerosis (9 2%), gastric cancer(5 1%) and normal tissues (2 5%). There were significantly differences among each groups(P<0 01), but between DU and GU(P>0 05). After eradication of HP ,AI was significantly decreased .NO levels were much higher in patients with HP(+) than those of healthy groups of HP( )(all P< 0 01). Therefore, there were no significantly differences among each groups of HP(+) (P>0 05). There were significantly positive correlated with NO and AI, but the correlation between in HP(+) gastric precancerosis and gastric cancer groups was not observed. Conclusions ⑴HP may induce apoptosis of gastric epithelia cells, it may be one of the mechanisms of HP leading to gastric cancer. After eradication of HP,the apoptosis is diminished. These may contribute to prevent the formation of gastric cancer . ⑵At the early stage of HP infection, apoptosis may be induced by persistent and abundant production of NO,so that eradication of HP,in order to reduce the epithelial apoptosis and could prevent gastric cancer occurrence.
