贫血患者红细胞生成素水平与血红蛋白浓度关系的研究

摘　　要：目的 :探讨贫血患者红细胞生成素 (Epo)水平与血红蛋白 (Hb)浓度的关系。方法:采用ELISA法对162例不同原因的贫血患者测定了血清Epo水平 ,同时用常规方法测定Hb和血尿素氮 (BUN)。结果:按BUN水平划分的3组肾性贫血患者 ,BUN之间和Hb之间均有显著性差异 (F=94.80 ,P<0.001;F=17.03 ,P<0.001) ,而Epo水平之间无显著性差异 (F=1.46 ,P>0.05)。肾性贫血患者BUN、Hb、Epo水平相关分析显示 ,Hb与BUN存在显著负相关 (r= -0.6981,P<0.001) ,而Epo与BUN和Hb均无相关性 (r分别为0.1116和 -0.2001 ,均P>0.05)。非肾性贫血患者在可比的肿瘤性和增生性两类贫血均按贫血程度所分的3组患者中 ,Hb浓度间均有显著性差异 (F=129.53 ,P<0.001;F=80.1772,P<0.001) ,而Epo水平间均无显著性差异 (F=2.20 ,P>0.05 ,F=0.4996,P>0.05)。非肾性贫血患者Epo和Hb水平相关分析显示 ,肿瘤性贫血Epo与Hb存在负相关(r= -0.3526 ,P<0.05) ,而慢性病贫血和增生性贫血Epo与Hb均无相关性 (r=0.0658,P>0.05;r= -0.0132,P>0.05)。结论:肾性贫血患者Epo水平与Hb浓度无相关性 ,其贫血程度虽然主要取决于Epo水平 ,但它不是唯一因素。绝大多数非肾性贫血患者Epo水平与Hb浓度也无相关性 。Objective:To explore the relationship between erythropoietin(EPO) level and hemoglobin(Hb) concentration in patients with anemia.Methods: Serum EPO level of 162 cases with anemia due to different causes were measured by ELISA .At the same time, the blood urea nitrogen (BUN) and Hb concentration were determined by conventive method. Results: There were significant differences of BUN and Hb concentration among 3 groups patients with renal anemia (divided by BUN level)(F=94.80,P<0.001;F=17.03,P<0.001), while no significant differences of Epo level (F=1.46,P>0.05). Correlative analysis of BUN , Hb and Epo level in renal anemia patients suggested that there are obvious negative correlation of Hb and BUN concentration (r=-0.698 1, P<0.001),while no correlativity of Epo with BUN as well as Hb concentration(r=0.111 6 and-0.200 1 respectively, all P>0.05) .The non-renal anemia (tumorous and hyperplastic anemia)patients were divided into 3 groups according to anemic degree , there were obvious differences in Hb concentration among those 3 groups (F= 129.53,P<0.001;F=80.177 2 , P<0.001),while there are no obvious differences of Epo content (F=2.20,P>0.05;F=0.499 6,P>0.05 ) .The correlative analysis on Epo and Hb level in non-renal anemia patients indicated that there are negative correlation(r= -0.352 6, P<0.05) of Epo with Hb level in patients with tumorous anemia, while no correlativity of Epo and Hb level in patients with chronic and hyperplastic anemia (r= 0.065 8, P>0.05; r=- 0.0132,P>0.05 ).Conclusion:There are no correlativity between the Epo and Hb concentration in renal anemia patients. Although the Epo level is an important factor of anemia severity,yet it is not a sole factor.There are no correlativity of Epo and Hb concentration in most non-renal anemia patients, but the patients with tumorous anemia can present negative correlation.
