补气活血方药对冠心病血瘀证单核细胞凝血、纤溶活性作用研究

摘　　要：目的 :比较补气活血方药对冠心病血瘀证患者单细胞凝血、纤溶活性的作用 ,探讨治疗冠心病的有效方法。方法 :冠心病血瘀证患者30例 ,采用发色底物法分别检测细菌脂多糖(LPS)刺激前后和加用参麦注射液、川芎嗪注射液干预后外周血单核细胞(PBMC)促凝活性(PCA)、组织型纤溶酶原激活物(t-PA)及其抑制物(PAI 1)活性 ,比较其变化。结果 :应用LPS刺激后PBMCPAI 1升高 ,PCA升高显著(P<0 01) ,t PA无变化(P>0 05) ;参麦针干预后 ,各项指标均未见改变(P>0 05) ,川芎嗪针能降低PBMCPCA表达(P<0 05) ,而对PA系统表达无影响(P>0 05) ;同时应用川芎嗪加参麦针干预则可降低PBMCPAI 1表达(P<0 05) ,显著降低PCA表达。结论 :活血药降低凝血活性的作用强于补气药 ,补气活血药同时应用明显优于单纯补气药或单纯活血药 ,补气活血标本兼治的方法是治疗冠心病血瘀证患者的有效方法。Objective:Compare the effects of formulars of tonifying Qi and moving blood on changes of lipopolysaccharide indeced(LPS) human peripheral blood monocyte(PBMC) coagulant and fibrinolytic activity in coronary heart disease(CHD) patients with blood stasis and search for effective way for curing CHD.Methods:30 cases of coronary heart disease with blood stasis,activity of tissue type plasminogen activator(t PA) and its inhibitor(PAI 1)and procoagulant activity (PCA) of PBMC were measured by chromogenic substrate method respectively before and after LPS stimulation and formula treatment.Results:PAI 1 activity and PCA expression of PBMC were significantly increased after LPS stimulation (P<0 05,P<0 01) while t PA activity of PBMC unchanged.There was no significantly index changes after Shenmai injection treatment.PCA expression of PBMC was lowered after Chuanxiongqing injection treatment (P<0 05) while PA system was found no significant changes (P>0 05).PBMC PAI 1 activity expression was lowered (P<0 05) and PCA was significantly lowered (P<0 01) when Shenmai and Chuanxiongqing were used simultaneously.Conclusions:Function of lowering monocyte coagulant activity in moving blood formulars is superior to tonifying Qi formulars.Using moving blood formulars and tonifying Qi formulars together is superior to using separately.Tonifying Qi and promoting blood circulation is the effective way for curing CHD.
