扶正解毒祛瘀法对骨髓增生异常综合征转白因素的影响

摘　　要：目的:探讨扶正解毒祛瘀法对骨髓增生异常综合征（MDS）向急性白血病转化（转白）影响因素的治疗作用。方法:应用自拟清毒片、养正片、瘀毒清治疗MDS18例,将RA、RAS拟定为低危组,RAEB、RAEB-t拟定为高危组,观察治疗前后低危组、高危组血清白细胞介素2（IL-2）和可溶性白细胞介素2受体（SIL-2R）活性变化,低危组转白积分变化,高危组骨髓原始细胞数变化。结果:2组治疗前与正常人组比较,血清IL-2活性明显降低,SIL-2R活性明显升高（均P<0.01）;高危组和低危组治疗前比较,高危组血清IL-2活性明显降低,SIL-2R活性明显升高（均P<0.01）;2组治疗前后自身比较,血清IL-2活性均明显升高,SIL-2R活性均明显降低（均P<0.01）。低危组转白积分治疗前后比较,治疗后明显降低（P<0.01）;高危组骨髓原始细胞数治疗前后比较,差异无显著性意义（P>0.05）。结论:扶正解毒祛瘀法对MDS转白影响因素有一定的治疗作用,可以降低MDS白血病转化率。ve: To explore the effect of healthy - qi sup-porting, toxic - substance removing and blood - stasis eliminatingtherapies (HTBT) on acute leukemia transformation factor (LTF) in myeloproliferative disorder syndrome (MDS) . Methods: 18 cases of MDS were treated by Qingdu Pian, Yangzheng Pian and Yuduqing, RA and RAS were considered as low danger group (OLDG) and RAEB and RAEB - t as high danger group (HDG). Activity of IL - 2 and soluble interleukin - 2 receptor(SIL - 2R) of both groups and the leukemia transformation score of LDG and the number of merrow primary cell in HDG before and after treatment were de tect-ed. Results: Compared with normal subjects, the activity of IL - 2 was markedly decreased and the activity of SIL - 2R markedly increased ( P < 0. 01) before treatment in both groups. The activity of IL - 2 was markedly decreased and the activity of STR - 2 markedly in-creased in HDG before treatment(P< 0.01) as compared with LDG. The autologous comparison before and after treatment between 2 groups showed that the activity of IL - 2 was markedly increased and the activity of SIL - 2 markedly decreased ( P < 0. 01) in the same individual after treatment in both groups. The leukemia transformation score was marked decreased after treatment in LDG( P < 0. 01) but the merrow primary cell count showed no significant difference after treatment ( P > 0. 05). Conclusion: The HTBT possesses certain ther-apeutic effedt on LTF of MDS, and can lower the leukemia transfor-mation rate in MDS.
