改良耻骨上前列腺切除术的临床应用

摘　　要：采用自行设计的耻骨上前列腺切除术 (下称改良术 )对 2 12例前列腺增生症 (BPH)患者行前列腺切除。术中前列腺切除后只间断缝合颈口 12点 1～ 2针 ,然后立即采用尿管水囊牵引压迫止血 ,将出血封闭在前列腺窝内 ,形成一个有利于止血的封闭空间 ,无需花费时间特殊止血。手术时间平均 18分钟 ,术中出血平均 85 ml,均未输血 ,均不置膀胱造瘘管。术后膀胱冲洗时间平均 19.5小时 ,冲洗液用量为 15 0 0～ 30 0 0 ml;拔除尿管后均排尿通畅 ;尿流率、残余尿、生活质量和前列腺特异性抗原 (PSA)较术前均有明显改善。认为改良术是一种快捷、简便、高效。212cases BPH were conducted with the new methods of prostatectomy designed by ourselves(the modified prostatectomy).Only 1～2 acuses were sutured on the 12 o'clock point of bladder neck after the excision of prostate.Outlet of bladder neck were oppressed to stanch with balloon of Foley catheter.No more time was to stanch because the cavity of prostate became tight. All cases were operated successfully with the mean 18 minutes,bleeding 85ml,and no blood transfusion.After operation,the mean time of irrigation of bladder was 19.5 hours,and the irrigation liquid was 1500～3000ml without cystostomy. All patients voided fluently after the catheter was removed. Uroflowrate , residual urine,life quality and PSA were improved significantly after operation.The modified prostatectomy is a fast, simple and effective methods and popularized worthfully.
