脾大部切除胸腔移位加断流术治疗门静脉高压症疗效观察

摘　　要：对 34例肝硬变门静脉高压症患者行脾大部切除胸腔移位加贲门周围血管离断术 ,并进行远期随访。结果患者术后门静脉压力明显降低 (P<0 .0 5 ) ;外周血红细胞、白细胞和血小板数升高 ;术后 1年 B超复查显示残脾无明显增大及缩小 ,钡餐检查显示食管胃底静脉曲张改善或消失。随访 1～ 8年 ,术后 1年、3年和 5年生存率分别为 97.1%、94 .2 %和 88.3%。34 patients with hepatic cirrhosis and portal hypertension were followed up after subtotal splenectomy with thoracic cavity splenic transposition plus pericardia devascularization. Results showed that the free portal venous pressure decreased significantly (P<0.05) and peripheral RBC, WBC and PLT were increased after operation.In the first year after operation,the residual spleen did not enlarge or contract obviously showed by ultrasonography, esophageal and gastric fundus varices disappeared or improved turned out by gastroesophagram. All patients were followed up for 1～8 years, the 1,3 ,and 5 year survival rates were 97.1%,94.2% and 88.3% respectively. These suggest that the operation is effective for portal hypertension due to hepatic cirrhosis.
