摘 要:目的 探讨血浆纤维连接蛋白、尿纤维连接蛋白和尿转铁蛋白在糖尿病肾病 (DN)和糖尿病视网膜病变 (DR)的变化和发生机制。方法 检测 6 2例 2型糖尿病患者 (其中伴有 DN者 38例 ,伴有 DR者 14例 )和15例健康对照者的 2 4小时尿白蛋白排泄率 (AER)、纤维连接蛋白 (U Fn)、尿转铁蛋白 (U Trf)、β2 -微球蛋白 (Uβ2 -MG)、α1-微球蛋白 (Uα1 - MG)和血浆纤维连接蛋白 (PFn)水平变化。结果 2型糖尿病组 PFn水平与健康对照组无明显差别 (P>0 .0 5 ) ,伴有 DN和 DR的糖尿病患者与未发现 DN和 DR的糖尿病患者相比 ,其 PFn水平略有升高 ,但无显著性差异 (P>0 .0 5 ) ;伴有 DN的 2型糖尿病患者 UFn和 UTrf水平较未发现 DN的糖尿病患者显著升高(P<0 .0 1) ;伴有 DR的 2型糖尿病患者 UFn水平升高不明显 (P>0 .0 5 ) ,但 U Trf水平升高有统计学意义 (P<0 .0 1)。PFn与 U Fn、UTrf、AER、Uβ2 - MG和 Uα1 - MG无明显相关关系 ;而 UFn和 U Trf与 AER、Uα1- MG、Uβ2 -MG呈显著正相关。结论 UFn,UTrf的升高能早期反映 DN的出现 ,而 UTrf升高可能反映 DR的发生。Objective To investigate the change of plasma fibronectin (PFn), urinary fibronectin (UFn) and urinary transferrin(UTrf) in the type 2 diabetic patients with nephropathy (DN)and retinopathy(DR). Methods The levels of PFn, UFn, UTrf, urinary albumin exertion rate (AER), urinary β 2-microglubulin (Uβ 2-MG) and urinaryα 1-microglubulin(Uα 1-MG) were measured in 62 type 2 diabetic patients(including 38 DN and 14 DR) and 15 healthy controls. Results No significant changes of PFn were found in type 2 diabetic patients when they were compared with normal controls(P>0.05). The PFn levels of diabetics with nephropathy and retinopathy were higher than those of diabetics without nephropathy (DWN) and retinopathy (DWR) respectively, but there was no statistical significance (P>0.05). The excretion of UFn increased significantly in diabetics with nephropathy (P<0.01), but no significant change of UFn was found in diabetics with retinopathy(P>0.05). The level of UTrf elevated significantly in diabetics with nephropathy and retinopathy also (P<0.01). There was no relationship between PFn and UFn, UTrf, AER, Uβ 2-MG, Uα 1-MG, but there was positive relationship between UFn and UTrf, AER, Uβ 2-MG, Uα 1-MG. Conclusion High levels of UFn and UTrf can reflect diabetic nephropathy early. Increased UTrf may reflect diabetic retinopathy.
