摘 要:目的 了解 α受体阻滞剂酚妥拉明在犬腹部手术并发急性肺损伤 (AL I)中的作用 .方法 将实验动物随机分为腹部手术组 (A组 )、腹部手术加油酸损伤组 (B组 )、腹部手术加油酸损伤加酚妥拉明干预组 (C组 )、腹部手术加油酸损伤加山莨菪碱干预组 (D组 ) ,用检测丙二醛 (MDA)、超氧化物歧化酶 (SOD)、谷胱甘肽 (GSH)和谷胱甘肽过氧化物酶(GSH- PX)来评定氧自由基水平 ,取右肺组织做病理学切片检查 ,观察酚妥拉明对损伤肺嗜中性粒细胞聚集的影响及对氧自由基水平的干预作用 .结果 D组的 MDA (8.8± 1.7)μmol· L- 1明显低于其他各组 (P<0 .0 1) ,C组与 B组的 SOD[(2 13± 2 2 ) vs(2 11± 13) k U· L- 1 ]明显低于 A和 D两组(P<0 .0 5 ) ,D组 GSH(1.0 3± 0 .15 ) mmol· L- 1 明显高于其他各组 (P<0 .0 1) ,B组的 GSH- PX (92± 14 ) U· L- 1 低于其他各组 (P<0 .0 1) .周围血涂片嗜中性粒细胞计数由高至低依次为 :B,C,D和 A组 .病理切片见炎细胞浸润与肺泡充血水肿程度由重至轻依次为 :B,C,D和 A组 .结论 酚妥拉明能抑制嗜中性粒细胞在肺内聚集 ,减轻肺的局部损伤 ,但其作用较山莨宕碱弱 。AIM To study the effect of phentolamine (α receptor inhibitor) on dogs' acute lung injuries (ALI) concomitant with abdominal operation. METHODS Experimental animals were randomly divided into the abdominal operation group (Group A), the abdominal operation and oleic acid injury group (Group B), the abdominal operation and oleic acid injury and phentolamine intervention group (Group C) and the abdominal operation and oleic acid injury and anisodamine intervention group (Group D). The level of free radical of oxygen was evaluated through detection of MDA, SOD, GSH and GSH PX, and then by pathologic section biopsy of tissues in the right lung, the influence of phentolamine on aggregation of neutrophilic granulocytes and its intervention effects on the level of free radical of oxygen were observed. RESULTS MDA in Group D was obviously lower (8.8± 1.7 ) μmol·L -1 than those in other groups ( P <0.01), SOD in Groups C and B were obviously lower [(213±22) vs (211±13) kU·L -1 ] than those in Group A and D ( P <0.05), GSH in Group D was higher ( 1.03 ±0.15) mmol·L -1 than those in the other groups ( P <0.01), while GSH PX in Group B was obviously lower (92±14) U·L -1 than those in other groups ( P <0.01). The counts of neutrophilic granulocytes in peripheral blood smears from the highest to the lowest were B, C, D and A. Through pathologic section of cells, the degree for infiltration of neutrophilic granulocytes and alveolar hyperemia and hydrops from the severe to the mild were B, C, D and A. CONCLUSION Phentolamine can inhibit the aggregation of neutrophilic granulocytes inside the lungs and reduce the local lung injuries, but its effect is not as good as that of anisodamine and can be used to prevent and treat acute lung injuries during or after the operation.
