摘  要:目的 :观察人工全髋关节置换术后在假体周围的髋骨骨矿量 (BMC)骨密度 (BMD)的改变规律。方法 :采用DPX -L骨密度仪连续追踪调查 5 2例人工全髋关节置换术后 3、6、12、2 4和 36个月的髋骨假体周围的BMC和BMD ,以术前、后健侧髋为对照 ,进行分析研究。结果 :发现术后 3、6个月BMC和BMD均较术前下降 (P≤ 0 .0 5 ) ,患髋经 12~2 4个月运动练习 ,BMC和BMD逐渐增加 ,至 36个月时达到或接近健侧髋骨水平并获得稳定 (P≤ 0 .0 1)。结果显示BMC和BMD的改变与人工全髋关节的固定方式即骨水泥固定与生物型固定无明显关系 (P <0 .0 1)。结论 :人工全髋关节置换术后 6个月内BMC和BMD呈不同程度下降 ,患髋经 12~ 2 4个月运动练习 ,BMC和BMD逐渐增加 ,至 36个月时达到健侧髋骨的水平并获得稳定 ,与固定方式无明显关系。Objective: To research the significance and rule of the different changes of bone mineral content (BMC) and bone mineral density(BMD) in the replacement areas of hip periprothesis after total hip replacement(THR).Methods:To investigate BMC and BMD of 52 patients in the examined areas of hip periprosthesis with THR during 3,6,12,24and 36 months postoperation. It was analyzed and contrasted with the health side hip.Results:It was found that the BMC and BMD were decreased in three and six months after THR ( P ≤0.05),but it was gradually increased when they exercised with their hip joints from 12 to 24 months,and neared or had the lever of the normal,and steadness until 36 months ( P <0.01).The results were showed that there was no significant relationship of the changes of BMC and BMD with different modes of fixation for THR with cemented or biofixed ( P <0.01).Conclusion:There were a different decrease of BMC and BMD in the replacement areals of hip periprpthesis during six months after THR,but it increased in gradual when they exercised with their hip joints in 12 to 24 months ,and it was level of health side hip of BMC and BMD,and was no significant relationship of the changes of BMC and BMD with different modes of fixation for THR with cemented or biofixed.
