APAAP免疫组织化学技术在骨髓增生异常综合征骨髓病理中的应用

摘　　要：目的探索免疫组化技术应用于骨髓冷包埋切片检测骨髓增生异常综合征 (myelodysplasticsyndromes,MDS)红细胞系增殖及病态造血的可能性及应用价值。 方法碱性磷酸酶 抗碱性磷酸酶免疫酶标(alkalinephosphatseanti alkalinephosphatse ,APAAP)技术进行血型糖蛋白A(GlycophorinA)单抗分析 2 5例MDS患者的骨髓塑料冷包埋切片中有核红细胞增生程度及异常分布 (病态造血 ) ,以缺铁性贫血 (irondeficientdiseases,IDA) 14例作对照。结果MDS组每平方毫米骨髓面积中红系有核细胞数 (GlycophorinA阳性 )为 2 3～ 45 6个 /mm2 ,平均 (118.10± 83.5 9)个 /mm2 ,IDA对照组每平方毫米骨髓面积中红系有核细胞数为 2 3～ 10 2个 /mm2 ,平均(6 2 .35± 31.14)个 /mm2 ,有显著差异 (t’ =2 .6 0 ,P <0 .0 5 ) ;MDS 2 5例中有 2 3例 (92 %)出现幼红细胞簇异常分布 ,IDA 14例中对照组仅 3例 (2 1%)出现幼红细胞簇异常分布 ,u值 =3.0 4,P <0 .0 1。结论APAAP免疫组化技术应用于骨髓冷包埋切片可准确检测出红系造血细胞 ,MDS红系造血细胞增生较缺铁性贫血更旺盛并存在异常分布 ,此项指标可作为确诊MDS的有用手段之一。ObjectiveTo explore the possibility and the value of Immunocytochemical technique in GMA plastic cold embedded bone marrow sections to detect the hyperplasia and dyshematopoiesis of erythroblast in patients with myelodysplastic syndromes(MDS).MethodsGlycophorinA by APAAP technique was used on bone marrow cold embedded sections in 25 MDS patients to analysis the hyperplasia and abnormal localization(dyshematopoiesis) of nuclear erythrocyte with 14 cases of iron deficient diseases (IDA) as control.ResultsNuclear erythrocyte(glycophorina A,positive) of the MDS group ranges form 23～456/mm 2,with average 118.10 ± 83.59 /mm 2.It showed great discrepancy with the control group (IDA) in which the nuclear erythrocyte count was 23～102/mm 2(average 62.35 ± 31.14 /mm 2 t'= 2.6 ,P< 0.05 ). Of 25 cases(92%)of MDS,23 cases showed abnormal localization of proerythrocyte clusters while the control group only 3 of 14 cases(21%).Showed such cnahges (P< 0.01 ).ConclusionErythrocyte could be detected correctly by immunocytochemical technique applied onto GMA plastic cold embedded sections.Hyperplasia of erythrocytes in MDS is much more than that in IDA and MDS has abnormal localization of erythrocyes (dyshematopoiesis).
