摘　　要：目的评价HOMA法胰岛素抵抗指数 (HOMA -IR)是否适合于中国 2型糖尿病胰岛素抵抗的临床应用。　方法同步采用HOMA法和血糖钳夹试验对 48例 2型糖尿病人的胰岛素抵抗进行相关分析。　结果HOMA -IR与血糖钳夹试验的胰岛素抵抗指数 (clamp -IR)明显相关 ,经对数转化的二者之间具有更强的相关关系。在亚组分析中 ,对数化的HOMA -IR与clamp -IR在非肥胖和肥胖组、男性和女性组、正常血压和高血压组、饮食疗法和药物治疗组之间均有类似的明显相关关系。经多元回归分析 ,对数化的HOMA -IR是所有与胰岛素抵抗相关的临床指标中最强的相关指数。　结论HOMA -IR适合于中国 2型糖尿病的临床应用。Objective To evaluate whether the homeostasis model assessment(HOMA) is a reliable index in Chinese type 2 diabetic patients. Methods In the present study, we compared insulin resistance index as assessed by the 2-h glucose clamp test with HOMA in 48 type 2 diabetic patients. Results HOMA-IR significantly correlated clamp insulin resistance index (clamp-IR). The logarithmic transformed HOMA-IR correlated more strongly with logarithmic transformed clamp-IR than did HOMA-IR. The degree of correlation between log HOMA-IR and log clamp-IR was very similar between the lean (BMI<25kg/m 2) and obese subjects, men and women, normal tensive and hypertensive subjects, diet ary and oral hyperglycemic agent intake subjects. In multiple regression analyses, log HOMA-IR was shown to be the strongest contributor to clamp-IR among the clinical variables. Conclusion HOMA-IR strongly correlated with clamp-IR in Chinese type 2 diabetic patients. It can be reliably used in clinical practice.
