胫骨带锁髓内钉治疗骨折与锁钉装置的改进

摘　　要：目的　探讨应用带锁髓内钉治疗胫腓骨骨折 ,在不需X线引导下进行手术的可行性和效果。方法　应用非扩髓带锁髓内钉治疗胫骨骨折 46例。其中闭合性骨折 2 5例 ,开放性骨折 2 1例。改进带锁髓内钉远端瞄准器 ,增设辅助定位装置。结果　全部病例经随访 6个月以上 ,除 1例骨折术后1 0个月愈合外 ,45例骨折愈合时间平均 4 8个月。患者膝、踝关节功能均恢复正常。改进后的带锁髓内钉远端瞄准器 ,一次钻孔交锁成功 45例 ,无锁钉放置失败 ,术后无髓内钉或锁钉断裂。结论　非扩髓带锁髓内钉治疗胫腓骨骨折 ,手术不需X线引导 ,改进后的带锁髓内钉远端瞄准器 ,锁钉准确 ,简便实用。Objective To explore a method of treatment of fracture of the tibia using intramedullary interlocking nail improved with a distal screwing device Methods Forty six cases of fracture of the tibia were treated with the non reaming interlocking nail,which was improved with the distal localization and orientation devices.There were 25 cases of closed and 21 open fractures.Results The follow up period lasted for more than 6 months;45 cases had fracture union in an average of 4 8 months;one had delayed union.The functions of the knee and ankle joints resumed to their full extent.The insertion of the nails were successful at the first attempt and there was no complications.Conclusion Treatment of fracture of the tibia with the non reaming interlocking nail improved with the distal devices of localization and orientation is simple and effective.The treatment can be carried without the aid of X ray guidance.<
