不同起搏方式对心脏起搏患者心理状态的影响

摘　　要：目的 :对安装埋藏式心脏起搏器的患者采用量表化分析 ,观察不同起搏方式对心脏起搏患者的心理特点的影响。方法 :病例组 6 0人 ,均由经静脉系统植入埋藏式心脏起搏器 ,时间超过 3个月。其中房室顺序起搏组 (安装DDD或VDD型起搏器 ) 2 4例 ,心室单腔起搏组 (安装VVI型起搏器 ) 36例。在常规随访过程中要求患者填写完成问卷或量表 ,包括一般状况调查、患者心脏症状调查表、心理健康水平评定量表 (90项症状自评量表SCL 90 )。对照组为门诊查体的健康人 30例 ,按要求填写SCL 90量表。量表采用标准评分法进行统计分析。结果 :房室顺序起搏组和心室单腔起搏组患者的心理状态水平均低于对照组。房室顺序起搏组SCL 90得分与心室单腔起搏组比较差异有显著性 ,前者优于后者。心室单腔起搏组患者抑郁、敌对性、躯体化等心理症状得分高于房室顺序起搏组 (P <0 .0 5 )。结论 :不同心脏起搏方式可以影响长期心脏起搏患者心理状态 。Object: To analyze the psychological condition of patients with a cardiac pacemaker with measuring scale and to observe the influence of different pacing modes on the patients . Method: 60 patients were selected randomly from our paced patients with DDD or VDD pacemaker implanted,in 24 and VVI pacemaker in others.Three months or more after implantation , follow-up treatment was provided and the patients were asked to fill in questionnaires and scales investigating the general condition , the cardiac symptoms and psychological condition (with SCL-90). A comparison group of 30 healthy persons, matching in age ,sex and education were also asked to fill in the SCL-90 . Results: The psychological levels of DDD/VDD group and VVI group were all lower those of the comparision group. Significant differences were found in SCL-90 scores between the DDD/VDD and VVI groups,the former being better than the latter .The degree of depression,rivalry and other symptoms were higher in the ⅤⅥgroup than in DDDⅣDD prouop (P<0.05). Conclusion: Different pacing modes produce different influence on the psychological condition of patients with a cardiac pacemaker,and the patients' psychological condition of DDD/VDD group were better than those of VVI group.
