Ilizarov外固定架肢体延长的临床应用分析

摘　　要：为探讨 Ilizarov外固定架肢体延长术在临床上的应用和相关问题 ,重点讨论其理论基础、技术操作要点及并发症。对采用Ilizarov外固定架进行肢体延长的 34例患者进行总结分析。结果延长最长 13cm,最短 2 cm,平均 5 .4 cm;延长时间 18～ 14 5天 ,平均 5 8天 ;愈合指数 30～ 4 0天· cm- 1 ,平均 36天·cm- 1。 34例均达到预期延长长度 ,其中 1例发生胫神经损伤 ,8例出现针道无菌性炎症反应 ,6例出现膝关节屈曲挛缩畸形 ,4例出现马蹄畸形 ,经治疗均康复。认为 Ilizarov肢体延长技术是以张力 -应力为理论基础的 。In order to study the clinical application of ilizarov external fixation on extremity prolonged operation, thirty four patients with ilizarov external fixation on extremity prolonged operation were analyzed. The results showed that maximum prolonged length was 13cm, minimum prolonged length was 2cm(average 5.4cm), time was 18 145 days(average 58 days), union index was 30 40 days/cm(average 36 days/cm). Thirty four patients all reached the expected length. Tibial nerve injury occurred in one case, pin track aseptic inflammation occurred in 8 cases, knee joint flexion contracture occurred in 6 cases, talipes equines occurred in 4 cases. After treatment they all cured. It was suggested that ilizarov external fixation is valuable in extremity prolonged operation.
