摘 要:目的 探索一种能精确诊断踝关节前后脱位的X线量化分析指标。 方法 根据生物力学与几何学原理 ,设计一种能在踝关节侧位X线片上进行量化分析的“距骨中心中轴距”(L值 )测量方法 ;筛选 2 0 0名健康成人进行L正常值范围测定 ,并对 1 865例 1 92 0侧踝部损伤病例做回顾分析。 结果 2 0 0名健康成人的L值为 0 .0~ 2 .5mm[(0 .84± 0 .59)mm] ,其 95 %位数 (P95)为 1 .99mm ;以L值 >2mm为诊断标准 ,踝关节前后脱位组的阳性诊断率即诊断灵敏度为 1 0 0 .0 %。结论 L值可望作为踝关节前后脱位的一个精确诊断标准 。Objective To explore an X ray quantification analysis index helpful for diagnosis of anteroposterior dislocation of the ankle joint. Methods According to the principle of biomechanics and geometry, we made a measurement technique, ie, the distance from the center of astragalus to mean axis of tibia (L value), which could make quantification analysis in lateral ankle roentgenograms. Selective 200 normal adult persons were used to determine the reference ranges of L value. A retrospective analysis was done on 1 920 ankle injuries from 1 865 patients. Results The L value of 200 normal persons was 0.0 mm 2.5 mm (0.84± 0.59) mm, and the 95 percentile (P95) was 1.99 mm. Using L>2 mm as diagnosis criterion of anteroposterior dislocation of the ankle joint, we found that the sensitive rate of diagnosis was 100% in the group of anteroposterior dislocation of the ankle joint. Conclusions The L value can be regarded as an exact diagnosis criterion of anteroposterior dislocation of the ankle joint. It is worthy to do more clinical researches.
