摘  要:目的 探讨骨关节炎关节软骨中基质金属蛋白酶 (MMPs) 1、2、3及其抑制物 (TIMPs) 1、2的免疫组化表达及与软骨退变的关系。方法  2 2例因骨关节炎行关节置换的软骨组织常规HE染色观察其组织学变化 ,ABC免疫组化法观察关节软骨MMP 1、2、3和TIMP 1、2的表达 ,经图像分析研究MMP 1、2、3和TIMP 1、2与软骨组织学退变的关系 ,5例因股骨颈骨折行关节置换的软骨标本为正常对照。结果 骨关节炎关节软骨出现纤维化、裂隙、变薄 ,软骨细胞数量减少 ;MMP 1、3和TIMP 1在正常软骨无表达 ,在中度退变软骨中表达明显升高 ,在重度退变软骨中表达降低 ,TIMP 1与MMP 1表达升高不平行 ;MMP 2和TIMP 2在正常软骨低表达 ,在中度退变软骨中表达升高 ,在重度退变软骨表达降低 ,但TIMP 2表达升高。结论 在骨关节炎中MMP 1、2、3和TIMP 1、2表达异常 ,引起细胞外基质异常降解 。Purpose. To investigate the relationship between immunohistochemical expression of matrix metalloproteinase(MMPs)-1,2,3/their tissue inhibitor of metalloproteinase (TIMPs)-1,2 and the degeneration of cartilage in osteoarthritis. Methods. The histological changes of degenerated cartilages by haematoxyllin-eosin staining and the immunohistochemical expressions of MMPs-1,2,3 and TIMPs-1,2 were studied in 22 osteoarthritis cases. The image analysis revealed the relationship between the expressions of MMP-1,2,3, TIMP-1,2 and the degeneration of cartilage. Results. The osteoarthritis cartilage underwent fibroplasia, tearing and thinning. The quantity of chondrocyte decreased. The expressions of MMP-1,3 and TIMP-1 were not observed in normal cartilage, while increased significantly in moderate osteoarthritis and decreased in end-stage osteoarthritis. The expression of TIMP-1 was not paralleled with that of MMP-1,3. Little immunostaining of MMP-2 and TIMP-2 was seen in normal cartilage, while their expressions increased in moderate osteoarthritis and decreased in end-stage osteoarthritis. In end-stage osteoarthritis, the expressions of TIMP-2 was stronger than that of MMP-2. Conclusions. The abnormal expressions of MMP-1,2,3 and TIMP-1,2, which resulted in degradation disorder of extracellular matrix, could induce degeneration of cartilage.
