摘  要:目的:评价原发性高血压左心室不同功能状态和肥厚的 ACE 基因多态性频率分布,探讨ACE 基因多态性对原发性高血压左心室功能和肥厚的影响。方法:采用超声技术评价左心室结构(左心室室壁厚度)和收缩功能指标(射血分数)。分别依据左心室室壁厚度(正常与增厚)、收缩功能(正常、亢进和减低)不同状况分组。PCR 反应检测不同分组组内 ACE 基因多态性并计算其频率分布。结果:原发性高血压患者 ACE 基因型中 DD 型频率分布与左心室肥厚无关;Ⅱ型频率分布在左心室肥厚组高于非肥厚组,P=0.027。左心室收缩功能正常组与功能异常组之间,ACE 各基因型频率分布差异无显著性意义;收缩功能异常组内收缩功能亢进与功能减低亚组间Ⅱ基因型频率分布差异有显著性意义,P<0.05。结论:①左心室收缩功能亢进与收缩功能减低时 ACEⅡ基因型频率分布存在差异,表明 ACE 基因在收缩功能不同异常状态中可能存在不同效应;②ACEⅡ基因型频率在左心室肥厚组中增高可能与研究对象年龄和药物易感性等因素有关。Objective:This research will focus on the evaluation of the frequency distribution of poly- morphism of ACE gene in different types of left ventricular function and left ventrieular hypertrophy in es- sential hypertension.Methods:Echocardiographie techniques were used for the measurement of end-dias- tohc thickness of left ventricular wall and the ejection fraction of the left ventricle.According as the pa- rameters of cardiac anatomic structure and function,the patients were separated into non-hypertrophy group,hypertrophy group,normal systolic function group,hyper-systolic function group and hypo-systolic function group .PCR techniques were used for the detection of polymorphism of ACE gene .The frequency distribution of Ⅱ,ID and DD ACE genotypes was counted subsequently.Results:No significant frequency distribution odds of DD between hypertrophy group and non-hypertrophy group.The frequency distribution of Ⅱ in hypertrophy group was higher than non-hypertrophy groups'(P=0.027).There were no signifi- cant odds of frequency distribution of ACE genotypes between normal systolic function group and the ab- normal systolic function group.The frequency distribution of Ⅱ in hyper-systolic function group was high- er than hypo-systolic function group'(P<0.05).Conclusion:The frequency distribution odds of Ⅱ be- tween hyper-systolic function group and hyposystol'c function group demonstrated that a different effect of ACE polymorphism might exist in different systolic function status of the left ventricle.This primary results indicated that a higher frequency distribution of Ⅱ in hypertrophy group may be related to the old age and ACE inhibior sensitivity of the patient for the ventricular structure modification.
