SOCON内固定术和三种融合方法治疗腰椎滑脱

摘　　要：目的 :观察运用SOCON内固定技术加上后外侧横突间植骨、椎体间植骨和椎体间Prospace界面融合器三种融合治疗腰椎滑脱的临床疗效。方法 :从 1995年 10月～ 2 0 0 0年 9月 ,共 46例 ,用SOCON脊柱内固定行复位和固定 ,2 4例椎体间Prospace界面融合 ,4例行椎体间植骨融合 ,18例行小关节后外侧横突间植骨融合。结果 :45例Ⅰ°、Ⅱ°滑脱患者 ,全部达到解剖复位 ,1例Ⅲ°滑脱复位 70 % ,随访 6～ 60个月 ,平均 2 8个月。X线片显示 46例全部融合 ,未形成假关节 ,复位和椎间高度无丢失 ,未出现螺钉松动和折断征象。临床症状 :优 18例 ,良 2 6例 ,可 2例。结论 :SOCON内固定系统具备复位良好、固定确定。三种融合方法均取得满意融合率。Objective: To evaluate the clinical results of three kinds of fusion methods(intertransverse bone graft fusion posterlateral? intervertebral bone graft fusion and intervertebral Prospace fusion device)plus SOCON internal fixation used in the treatment of lumbar spondylolysis. Method: From Oct. 1995 to Sep.2000,the authors studied 46 cases, according to Newman classification, 39 cases of spondylolysis, 17 cases of isthmic defect spondylolysis. According to Meyerding classification, 29 cases of grade Ⅰ, 16 cases of grade Ⅱ, 1 cases of grade Ⅲ. The levels of spondylolysis 34 cases of L 4～5 ,12 cases of L 5～S 1. Decompression area included total laminectomy and bilateral nerve canal, SOCON pedicle devices were used in reduction and internal fixation, 24 cases of intervertebral Prospace fusion, 4 cases of intervertebral bone graft fusion, 18 cases of intertransverse bone graft postolateral fusion. Result:Anatomic reduction was achieved in 45 cases of grade Ⅰ?grade Ⅱ,70% reduction in 1 case of grade Ⅲ. All cases were followed up for 6～60 months, averaged 28 months. Radiography demonstrated total fusion in 46 cases, no pseudoarticulation, no failure in the maintenance of reduction and height of intervertebral space, no loosening or break of pedicle screws. The clinical signs showed 18 cases of excellent, 26 cases of good, 2 cases of fair. Conclusion: SOCON internal fixation system can achieve excellent reduction and firm fixation. Three types of fusion methods all showed satisfactory fusion rate.
