滤除白细胞对悬浮红细胞保存期质量的影响

摘　　要：目的探讨滤除白细胞对红细胞悬液在储存期内质量和功能的变化的影响。方法采集20名献血者400 mL新鲜全血,每份分为2份200 mL×2袋,将1袋制备成红细胞悬液作为对照组(n=20);另外1份使用白细胞滤器滤除白细胞后作为实验组(n=20),检测滤除白细胞后白细胞去除率和HGB回收率;(4±2)℃常规保存,分别在储存的0、7、14、21、28及35 d取样检测Na+、K+、CL-、ATP、2,3-DPG、乳酸、游离血红蛋白和储存末期溶血率。结果实验组白细胞去除率和HGB回收率分别为99.9%和92.49%;实验组和对照组K+、游离血红蛋白、储存末期溶血率和乳酸随着储存时间延长逐渐增高,前3者在储存21 d以后差异有统计学意义(P﹤0.05);乳酸在整个储存无显著差别;2,3-DPG随着储存时间延长逐渐降低,两组数据在整个储存期间也无显著差别;其他指标变化不显著。在储存时间的35 d内,两组所检测数据均在国家标准范围内。结论白细胞滤除对红细胞储存21 d以后的质量指标产生一定影响,但在可接受范围内。Objective To explore the effects of leukocyte filtration on the quality of banked red blood cells.Methods 400mL fresh blood from 20 healthy donors were equality divided into 2 aliquots,of which one was test group (with leucocytes removal),and the other was control group.Leukocyte removal rate and HGB recovery rate of test group were tested.Two groups were stored at(4 ±2)℃.Na+,K+,CL-, ATP,2,3-DPG,lactic acid,free hemoglobin,and hemolysis ratio were measured on day 0,7,14,21,28 and 35,respectively.Results Leuko-cyte removal rate and HGB recovery rate of test group was 99.9%and 92.49%,respectively.K+,free hemoglobin,hemolysis ratio and lactic acid were higher with time prolonging in two groups.The first 3 were obviously different between the two groups after day 21(P # 0.05).2, 3-DPG decreased gradually as the storage time prolonged,but there was no difference between the two groups.All test data were within the scope of the national standard.Conclusion Leukocyte filtration has some effects on the quality of red blood cells that are stored 21d and more.However,these effects are within the acceptable range.
