腹膜前间隙与疝环充填式无张力疝修补术疗效分析

摘　　要：目的分析腹膜前间隙修补术和疝环充填式无张力修补术治疗腹股沟疝的临床疗效。方法选取96例腹股沟疝患者为研究对象,按手术方式不同分为两组。试验组48例采用腹膜前间隙疝修补术治疗,对照组48例采用疝环填充式修补术治疗,比较两组疗效。结果两组术后均未出现切口血肿和血清肿、感染、膀胱及输精管损伤等,随访未出现复发、缺血性睾丸炎、睾丸萎缩、补片移位等。对照组患者疼痛持续时间长于试验组(P<0.05);对照组患者局部异物感发生率和伤口麻木感发生率均显著高于试验组(P<0.05)。两组手术时间、术中出血量、术后住院时间、术后下床活动时间、阴囊水肿、疝囊积液、尿潴留、伤口脂肪液化发生率的差异无统计学意义(P>0.05)。结论腹膜前间隙修补术与疝环充填式修补术均是安全有效的手术方式。腹膜前间隙修补术后,患者的生活质量更高,值得临床推广。Objective To compare and analyze the clinical efficacy of peritoneal clearance hernia repair and mesh- plug hernioplasty in the treatment of inguinal hernia. Methods A total of 96 patients with inguinal hernia from June 2013 to August 2014 received by our hospital were divided into two groups according to the different treatment methods; 48 cases of experimental group were given peritoneal clearance hernia repair,and 48 cases of control group were treated by mesh- plug hernioplasty,then the clinical treatment effect of the two groups was compared. Results There was no incision hematoma,seroma,infection and other early complications such as bladder or vas deferens damage occurred in both groups. No recurrence,ischemic orchitis,testicularatrophy,mesh displacement and other later complications were found as well. The pain duration in the control group was longer than that in the experimental group( P < 0. 05),and the local foreign body feeling incidence and wound numbness incidence in the control group were significantly higher than those in the experimental group( P <0. 05). However,the time of operation,blood loss,hospital stay as well as other perioperative clinical parameters between the two groups had no statistically significant difference( P > 0. 05). Conclusion The peritoneal clearance hernia repair and mesh- plug hernioplasty can be used as safe and reliable operation for treating inguinal hernia; however,the quality of life for patients who receive peritoneal clearance hernia repair is superior to the latter,so it is worth popularizing.
