手腕掌部毁损伤的显微外科修复

摘　　要：目的 总结近年来严重毁损伤修复的经验,采用带血管的髂骨移植再造掌腕骨及复合组织移植修复严重手腕掌毁损伤。方法 通过吻合血管保留残指,再造腕掌骨及复合组织修复使残指原位再植。结果 本组5例残指及复合组织全部成活,“再造手”外观良好,痛触觉恢复,能完成抓握及部分对掌功能。结论 对腕掌部毁损伤通过再造掌骨不但改善外观,功能恢复亦良好。Objective To improve the technique in restoration the function of sever damaged hand by reconstructing carpals and /or metacarpals with vascularized iliac bone grafting and combined tissue transfering according to our experience of recent years. Methods The digitals ware replanted to the original site by anastomosising or tranfering Vessals through ' bridge' of Kirschner wire between digitals and radials, the reconstructing carpal and/or metacarpals with vascularised iliac bone after two weeks when the digitals survived. Results Five cases of reconstructed hand were all survived with good appearence and excellent function of grasping. Conclution For severe carpal and/or metacarpals injury, the appeareance and fuction can be resusmed by reconstructing the carpals and /ormetacarpals with vascularized iliac bone graftion, on which digitals can be replanted.
