摘  要:目的研究不同平面髋臼横形骨折对髋关节稳定性的影响,以正确界定髋臼负重区及其手术指证。方法选24个尸体髋关节,按内侧顶弧角30°、40°、50°和60°截骨分为4组,每组6髋,以建立髋臼横形骨折模型。对骨折模型加载轴向载荷800N,分别测定反映各组髋关节稳定性的纵向、水平位移和刚度。结果加载800N时,30°、40°、50°和60°组髋臼的纵向位移分别是7.83mm、6.20mm、5.50mm和5.00mm,水平位移分别为0.036mm、0.029mm、0.024mm和0.022mm,轴向刚度分别为102.17N/mm、129.03N/mm、145.46N/mm和160.02N/mm。按照稳定性评判标准,30°和40°组分别有6髋和1髋完全不稳定,且40°组有3髋部份不稳定,50°组1髋部份不稳定,60°组无不稳定。结论后顶弧角大小与髋关节的稳定性密切相关,在内顶弧角>40°、后顶弧角≥45°且二者之和>90°时,能维持髋关节的稳定性。Objective To define the limited of the weight-b earing region of the acetabu lum with studied effects on the hip stability after th e different level transve rse acetab ular fractares.Methods Twenty four ca-daveric hip joints were divi ded into four groups:60,50,40and 30degrees of medial roof-arc a ngl e ,There are six hip joints in every group .The sim ulated transverse fracture models were created by osteotomy .Biome-chanical stability was evaluated on l ongitudinal displacement ,level displacement and axial stiffn ess measure-ment s ,Which were under the 800Newto n vertical compression load .Results Longi tudinal displacemennt in 30degrees group is 7.83mm,40degrees group is 6.20mm,50degrees group is 5.50mm and 60degrees group is 5.00mm ,level displacement in 30degrees,group is 0.036mm,40degrees group is 0.029mm,50degrees group is 0.025mm and 60degre es group is 0. 022mm ,axial stiffness in 30degrees group is 102.17N /mm,40degre es group is 129.03N /mm,50degrees group is 145.46N /mm and 60 degrees group is160.02N /mm .According to criteria o f stability ,there are 6and 1completely unstable in 30and 40degrees groups ,3and 1partial instable in 40and 50degrees groups ,respectively,there is no unstable joi nt in 60degrees group .Conclusions In transverse acetabular fractures,the s tabitity of the hip was signifi cantly ef-fected by a posterior roof-arc angl e and was maintained when a medial roof-arc angle greater than 40degrees,a p osterior roof-arc angle greater than or equate to 45degrees and a medi al pl us a posterior rool-arc angle greater than 90degrees.
