摘 要:目的 研究外周微循环白色微血栓和血液流变学变化与肾病综合征 (NS)并肾静脉血栓形成(RVT)之间的关系。方法 采用螺旋CT肾静脉连续薄层增强扫描 ,将 10 0例NS患儿分为Ⅰ组 (无RVT ,88例 )和Ⅱ组 (合并RVT ,12例 )。甲皱微循环下观察白色微血栓。血液流变学指标检测按常规方法 ,包括全血比粘度、血浆比粘度、红细胞电泳、血细胞压积。结果 1.Ⅱ组白色微血栓阳性率明显高于Ⅰ组 (P <0 .0 1)。 2 .Ⅰ组、Ⅱ组全血比粘度 (高切、低切 )、红细胞电泳时间、血细胞压积均显著高于对照组 (P <0 .0 0 1,P <0 .0 1) ;Ⅱ组全血比粘度 (高切、低切 )、红细胞电泳时间显著高于Ⅰ组 (P <0 .0 0 1)。Ⅰ、Ⅱ组血细胞压积无显著性差异 (P>0 .0 5 )。Ⅰ、Ⅱ组、对照组血浆比粘度无显著性差异 (P >0 .0 5 )。Objective To study the correlation between white microthrombus, hemorheology and renal vein thrombosis(RVT) in children with nephrotic syndrome(NS). Medthods One hundred NS children were divided into two groups, the study group (n=88) was NS patients without RVT and the control group (n=12) was NS children complicated with RVT. The RVT was diagnosed by thin layer scanning under spiral CT. White microthrombus were observed by examination of mail fold microcirculation and hemorheology by conventional method. Results 1.Positive rate of white microthrombus in the control group was much higher than that of the study group (P<0.01).2.The whole blood viscosity (high-shear, low-shear), erythrocyte electrophoresis time, packed cell volume were significantly higher in study group than those of control group(P<0.001);the whole blood viscosity (high-shear), erythrocyte electrophoresis time, packed cell volume were significantly higher in study group II than those in control group(P< 0.001); the whole blood viscosity (low-shear) and erythrocyte electrophoresis time in controls were significantly higher in study groupⅠthan that of control group(P<0.01) , The whole blood viscosity (high-shear, low-shear), erythrocyte electrophorsis time were significantly higher in control group than those of study group (P<0.001). No significant difference was found between two groups in packed cell volume. Conclusion The positive rate of white microthrombus and hemorheologic changes are closely correlated with RVT.
