摘  要:目的 研究高血压病患者血浆氧化修饰低密度脂蛋白 (oxLDL)的水平以及氯沙坦与硝苯地平单独和联合治疗对其的影响。方法  5 8例高血压病患者 ,2 1例接受氯沙坦 (Los)治疗 ,19例接受硝苯地平 (Ni)治疗 ,18例接受氯沙坦与硝苯地平联合 (Com)治疗 ,均为口服片剂 ,对照观察 6周。另选取与高血压病患者年龄、性别相匹配的正常对照15例。高血压病组治疗前后检测oxLDL、丙二醛 (MDA)。结果 高血压病患者治疗前oxLDL、MDA水平显著高于对照组 ,用药后Los组、Ni组较用药前oxLDL、MDA均有显著降低 (P <0 .0 5 ) ,Com组也显示出类似的趋势 (P <0 .0 1)。结论  1.高血压病患者体内存在着脂质过氧化修饰。 2 .氯沙坦和硝苯地平具有抗氧化作用 ,单独和联合治疗后oxLDL的降低 ,是它们抗动脉粥样硬化 (AS)的机制之一。氯沙坦在降压的同时 ,也可作为抗AS药物应用。Objective To study the level of plasma oxidize-modified low density lipoprotein (oxLDL) in essential hypertension (EH) and to investigate the effects of losartan or combined with nifedipine on plasma oxLDL in EH. Methods 58 patients were randomly divided into three groups:21 patients were treated with losartan(50~100 mg qd),19 patients with nifedipine(5~10 mg, bid~tid), 18 patients with combination of losartan and nifedipine (50mg qd and 5~10 mg qd) for 6 weeks. 15 sex and age-matched healthy served as control. oxLDL and MDA were examined before and after treatment. Results The levels of oxLDL and MDA before treatment in EH were markedly higher than those of controls. The levels of oxLDL and MDA of losartan group or nifedipine group were decreased significantly after treatment(P<0.05). An analogous trend of changes in oxLDL and MDA was shown in combination group as well(P<0.01). Conclusion Modification of lipidperoxide is present in EH. Both losartan and nifedipine seem to have the antioxidative capability. Treatment with losartan or combined with nifedipine, reducted oxLDL which may have the suppressive effection in atherosclerosis in EH.
