摘  要:目的 研究高血压病患者肺动脉压和肺动脉、肺静脉血流频谱变化。方法 常规经胸超声心动图检查 ,选择标准胸骨旁左室长轴切面、心尖四腔心切面等检查。以三尖瓣返流压差法估测肺动脉压 ;脉冲波多普勒检测肺动脉、肺静脉血流频谱 ;二维超声测各房室内径、左室壁厚度等。对各组数据进行统计学分析。结果 高血压病患者左室收缩功能代偿时 ,肺动脉瓣口前向血流速度增快 ,左室收缩功能失代偿后 ,肺动脉前向血流速度明显减慢 ;左室收缩功能不全时肺动脉压升高 ;肺静脉血流频谱S波峰值变小。结论 高血压病患者 ,左室收缩功能从代偿到失代偿的过程中 ,肺动脉瓣口前向血流存在着由快到慢的动态变化 ;并逐渐出现肺动脉压升高 ,肺静脉血流频谱异常。Objective To study the change of blood flow spectrum of pulmonary artery and pulmonary vein and pulmonary pressure in patients with essential hypertension. Methods Transthoracic echocardiography was routinely examined from left parasternal long-axis section and apical four-chamber section. Pulmonary pressure was measured by tricuspid regurgitation. Blood flow spectrum of pulmonary were measured by pulsed wave doppler echocardiography. The diameter of atrium and ventricle and the thickness of post-wall of left ventricle were measured by two-dimensional echocardiography. Results Blood flow velocity of pulmonary artery increased when left ventricular systolic function of the patients was in compensated situation. Blood flow velocity of pulmonary decreased and pulmonary pressure increased significantly when left ventricular systolic function was decompensated. Conclusion When left ventricular systolic function changed from compensation to uncompensation in patients with essential hypertension the blood velocity in pulmonary artery gradually decreased with pulmonary pressure increased and the hemodynamic of pulmonary vein was abnormal.
