乙酰螺旋霉素治疗弓形虫感染的临床疗效分析

摘　　要：为观察乙酰螺旋霉素治疗孕期与育龄妇女弓形虫感染的临床疗效及对宫内传播的影响。对 4 36 3名适龄妇女进行弓形虫感染筛查 ,并以弓形虫活动性感染的 12 2例育龄妇女和 6 7例孕妇为治疗组 ,给予乙酰螺旋霉素 0 .5 g/ 6 h,连服 10 d,以未做任何治疗的 2 8例孕妇和 2 2例育龄妇女感染者为非治疗组 ,比较两组弓形虫特异性 Ig M抗体的转阴率 ;取治疗组和非治疗组孕妇孕中晚期羊水和 (或 )脐血、产时胎盘组织进行弓形虫 DNA检测 ,比较两组孕妇宫内传播情况。结果显示 :孕妇与育龄妇女弓形虫近期感染率为 11.6 1%和 7.81% ,差异无显著性 ;治疗组孕妇和育龄妇女 Ig M转阴率分别为 6 7.16 %和 70 .4 9% ,而非治疗组的自然转阴率分别为 14 .2 9%和 13.6 4 % ,两组均有非常显著性差异 ;治疗组宫内传播率为 7.4 6 % ,非治疗组为 2 1.0 5 % ,差异有显著性意义 ;随访治疗后新生儿 ,未发现明显异常。提示 :乙酰螺旋霉素是治疗弓形虫感染的安全有效的药物 。The clinical effectiveness of acetylspiramycin in the treatment of toxoplasmosis infection and its effect on the uterine transmission were studied. In the treatment group, 67 pregnant women and 122 child bearing age women positive for toxoplasmosis IgM antibody were given acetylspiramycin 0.5 g/6 h for 10 days, and 28 pregnant women and 22 child bearing age women positive for toxoplasmosis antibody IgM served as control group. The negative rate of the Toxo IgM in the two groups was compared statistically after the treatment. The TOXO DNA was detected in amonioflouid, cord blood and placenta tissues in the two groups. The negative rate of Toxo IgM in pregnant women and child bearing age women after the treatment was 67 16 % and 70 49 % respectively, while in the control group, the negative rate was 14 29 % and 13 64 % respectively. There was a significant difference between the two groups ( P <0 01). The transmitting ratio to the fetus in the treatment group was 7 46 %, but 21 05 % in the control group. The difference between two groups was significant ( P <0 05). No obvious abnormalities were found in 46 newborns with their mother treated with acetyspiramycin. It was revealed that acetyspiramycin was a safe and effective chemical drug in treating toxoplasmosis infection during pregnancy.
