那屈肝素钙与硝酸异山梨酯或葛根素治疗不稳定性心绞痛的临床疗效

摘　　要：目的 :探索抗栓治疗基础上加用硝酸异山梨酯或葛根素治疗不稳定性心绞痛的临床疗效。方法 :将 5 5例不稳定性心绞痛 (UA)患者随机分为 :A组 30例 (男 2 1例 ,女 9例 ) ,年龄 (6 2 6± 12 9)a ,予那屈肝素钙 0 4～ 0 6mL ,sc,bid ,持续 7d ,硝酸异山梨酯 10mg +5 %GS 5 0 0mL ,ivgtt,qd ,持续 14d ;B组 2 5例 (男 17例 ,女 8例 ) ,年龄 (6 5 5 +10 3)a ,在那屈肝素钙治疗基础上加用葛根素葡萄糖注射液 2 5 0mL ,ivgtt,qd ,持续 14d。结果 :A组临床疗效的总有效率为 90 % ,与B组疗效 (6 0 % )相比 ,P <0 0 1;A组ECG改善 80 % ,优于B组 5 6 % (P <0 0 5 )。B组治疗后凝血酶原时间 (PT)及激活型部分凝血活酶时间 (APTT)均延长 ,而纤维蛋白原 (Fg)、血小板聚集率 (PAR)和血浆黏度 (PV)均下降 ;A组治疗后PAR及PV均下降 ,而PT ,APTT和Fg无显著变化。结论AIM: To study the effect of isosorbide dinitrate or puerarin intravenous drip in addition to antithrombotic therapy on unstable angina (UA). METHODS: Patients ( n =55) with UA were randomly divided into 2 groups: 30 patients [M 21, F 9; age (62 6±12 9) a] in group A received antithrombotic therapy (0 4-0 6 mL nadroparin calcium, sc, bid for 7 d) and isosorbide dinitrate 10mg in 5% glucose 500 mL, iv gtt, qd for 14 d; 25 patients [M 17, F 8; age (65 5±10 3) a] in group B received antithrombotic therapy plus glucose puerarin 250 mL, iv gtt, qd for 14 d. RESULTS: The total clinical effective rate in group A was 90%, which compared with the group B (60%) was obviously of great significance ( P <0 01). Electrocardiography improvement in the group A was 80%, which was also superior ( P <0 05) to that of the group B. In the group B, level of plasminogen time (PT) and activated partial thromboplastin time (APTT) prolonged, fibrinogen (Fg) decreased, platelet aggregation rate (PAR) and plasmic velosity(PV) lowered. In the group A, same changes in PAR and PV were shown but with PT, APTT and Fg unchanged. CONCLUSION: Both isosorbide dinitrate and puerarin in addition to nadroparin calcium therapy have good antithrombotic function and show good clinical effect in treating patients with UA.
