摘 要:目的 :探讨母亲对氧磷酶 2基因 [二乙基对硝基苯磷酸酯酶 2基因 (PON2 148) ]多态性位点和细胞色素P45 0IIE1基因 (CYP2E1)多态性位点对低出生体重的影响。方法 :采用病例对照研究方法 ,使用统一设计的调查问卷 ,由经过培训的调查员于 1998至 1999年在安徽省安庆市各县级医院对入院分娩孕妇及其单胎、活产的低出生体重儿和正常出生体重的对照新生儿进行调查 ,共得到有效样本 2 31个母亲新生儿对 ,其中低出生体重组 6 5对 ,正常出生体重对照组 16 6对。结果 :单因素分析结果显示 ,突变纯合子基因型 /杂合子基因型与野生纯合子基因型比较 ,未见CYP2E1多态性位点和PON2 148多态性位点导致低出生体重的危险性有显著性增加。多因素Logistic回归模型经母亲年龄、文化程度、生育史 ,新生儿性别、孕周混杂因素调整后 ,可见CYP2E1多态性位点和PON2 148多态性位点均导致低出生体重的危险性增加 (CYP2E1,OR =2 .4,95 %CI为 1.17~ 4.89;PON2 148,OR =2 .16 ,95 %CI为 1.0 6~ 4.41) ,并且有显著的统计学意义。经进一步分析CYP2E1多态性位点和PON2 148多态性位点之间对低出生体重的影响 ,其结果显示 :这两个多态性位点之间无明显交互作用 ,它们是低出生体重的独立危险因素。结论 :CYP2E1多态性位点和PON2 148多态性?Objective: To examine the association between polymorphisms of cytochrome P450 Ⅱ E1(CYP2E1) gene and Paraoxonase 2 (PON2148) gene on the low birthweight.Methods:Using standard questionnaire, the 231 singleton live born infant-mother pairs (cases:65 pairs; controls:166 pairs)from 1998-1999 at Anqing City in Anhui Province were investigated by trained field workers with epidemiologic case-control method.Results:Both CYP2E1 and PON2148 polymorphisms were not significantly associated with low birthweight, respectively, after adjustment for major confounders including maternal age, education, parity, passive smoking, infant gestational age and sex. They were significant respectively( CYP2E1, OR=2.4, 95%CI=1.17-4.89; PON2148, OR=2.16, 95%CI=1.06-4.41). When CYP2E1 and PON2148 polymorphisms were considered jointly, with CYP2E1 homozygous wild-type and PON2148 homozygous wild-type as reference, the low birthweight was significantly associated with CYP2E1 variant/mutation and PON2148 variant/mutation among women(OR=4.77, 95%CI=1.63-13.95), But we did not observe the gene interactions in relation to low birthweight. Conclusion: Our data suggest a genetic influence on low birthweight in this population, It is suggestied that there is no synergism of gene CYP2E1 and gene PON2148.
