老年冠心病和扩张型心肌病患者心力衰竭时血清可溶性细胞间黏附分子的检测

摘　　要：目的　观察老年冠心病和扩张型心肌病患者心力衰竭时血清黏附分子的变化。方法　采用固相酶联免疫吸附试验的夹心法 ,分别测定 38例冠心病心力衰竭和 16例扩张型心肌病 (DCM)心力衰竭时血清可溶性细胞间黏附分子 1(sICAM 1)和E 选择素。同时以同期 33例健康者作为对照组。结果　血清sICAM 1在冠心病组 (5 .6 0±0 .78)ng ml和DCM组 (5 .5 3± 0 .74 )ng ml均高于健康对照组 (4.0 1± 0 .39)ng ml;血清E 选择素在冠心病组 (44 .2 1±2 9.31)ng ml和DCM组 (43.80± 2 5 .6 6 )ng ml亦高于健康对照组 (2 3.4 2± 15 .84 )ng ml ,且心功能Ⅳ级组血清sICAM 1和E 选择性水平均高于心功能Ⅱ级组。结论　血清sICAM 1和E 选择素水平在心力衰竭时均升高 ,可能与动脉粥样硬化无明显相关 ;Objective To investigate serum adhesion molecule level in old patients with heart failure who have coronary heart disease and dilated cardiomyopathy.Methods Serum soluble intercellular adhesion molecules 1 (sICAM 1) and E selectin were measured with the commercially available enzyme linked immunosorbent assay method in 38 patients with coronary heart disease and heart failure,16 patients with dilated cardiomyopathy and heart failure,and 18 healthy control subjects.Results Serum sICAM 1 levels were higher both in patients with coronary heart disease [(5.60±0.78) ng/ml] and in those with dilated cardiomyopathy [(5.53±0.74) [CM)] ng/ml] than in control subjects than in control subjects [(4.01±0.39) ng/ml].E selectin levels were also higher in both groups with heart failure [coronary heart disease (44.21±29.31) ng/ml,dilated cardiomyopathy (43.80±25.66) ng/ml] than in control subjects [(23.42±15.84) ng/ml]).sICAM 1 and E selectin levels were higher in patients with class Ⅳ than in those with class Ⅱ and Ⅲ heart failure.Conclusions Serum sICAM 1 and selectin levels are increased in old patients with heart failure and are unrelated to the presence or absence of coronary heart disease,but they correlate with the severity of heart failure.
