2DE与LVG测定冠心病患者左室容积和射血分数的对比研究

摘　　要：二维超声心动图(2DE)心尖两切面对65例冠心病(CHD)患者左室舒张末容积(EDV)、收缩末容积(ESV)和射血分数(LVEF)的测定值与矫正双面左室造影(LVG)测值间均有良好的相关,r分别为0.89、0.92和0.82,P均<0.001;2DE对其中24例CHD伴有室壁节段运动异常(RWMA)者的EDV、ESV和LVEF测定结果与LVG测值间也均有良好的相关,r分别为0.82、0.86和0.82,P均<0.001。2DE所测LVEF与LVG正常和异常LVEF的符合率为85％。提示2DE对CHD患者的无创心功能定量测定是准确可靠的。The Comparison of left ventricular volumes (LVVs) and ejection fraction estimated by two-dimensional echocardiography (2DE) with left ventriculography (LVG) were carried out on 65 cases including 19 normal persons and 46 patients with coronary heart desease (CHD) among whom regional wall motion abnormality (RWMA) was demonstrated in 24 cases by LVG.Results: (1). There were close correlations between 2DE and LVG estimates of end-diastolic volume (EDV), end-systolic volume (ESV) and ejection fraction (LVEF) in total 65 cases, the r values were 0.89, 0.92 and 0.82 respectively (all P<0.001). Compared with LVG estimates, however, the mean EDV determined by 2DE was 4.9% lower (-8.4ml, P<0.05), mean ESV was 13.7% higher (10.3ml, P< 0.01) and mean LVEF was 14.1% lower (-8.7%, P<0.001). (2). There were linear correlations also between 2DE and LVG estimates of EDV, ESV and LVEF in 24 CHD patients with RWMA, the r values were 0.82, 0.86 and 0.82 respectively (all P<0.001). Compared with LVG estimates, the mean estimates of LVEF by 2DE was still 11.8% lower (-5.3%, P<0.01), while those of. EDV and ESV by 2DE both had no significant differences (both P>6.05). (3). The rate of agreement of 2DE LVEF with LVG normal and abnormal LVEF was 85%.It was indicated that 2DE is an accurate and reliable noninvasive method for quantitative assessment of left ventricular volumes and function in patients with CHD.
