摘  要:目的 研究血管紧张素转换酶 (ACE)基因、血管紧张素Ⅱ受体 1型 (AT1R)基因多态性及多种危险因素与高血压病 (EH)的关系。方法 采用整群抽样的方法抽取唐山开滦矿业集团职工10 4 3人 ,实际完成调查 10 0 7人。所有人群进行问卷调查、全面体检、测量血压。用PCR、限制性酶切、琼脂糖电泳分型的方法检测ACE、AT1R基因型。用SAS统计软件进行单因素、多因素分析。结果ACE基因、AT1R基因各基因型在正常血压组和EH组的分布差异无显著性。在与EH有关的各种危险因素存在的条件下 ,ACE基因DD型、AT1R基因AC +CC型EH患病率较高 ,但差异无显著性。两种基因的DD +AC联合基因型EH患病率高于其他基因型 ,差异显著。通过非条件logistic回归的筛选 ,与血压有关的因素有年龄、性别、打鼾、肥胖、EH家族史、心率、高甘油三酯血症 7项指标 ,ACE、AT1R基因及DD +AC联合基因型均未进入模型。结论 在其他危险因素存在的前提下 ,ACE基因的DD +AT1R基因的AC联合基因型人群患EH的危险性明显增加。但单一基因的作用是微小的 ,在EH的发病机制中 ,环境因素可能更重要。Objectives To investigate the relationship between polymorphism of angiotensin converting enzyme (ACE) or angiontensin II type I receptor (AT1R) genes as well as multiple risk factors and essential hypertension Methods A total of 1 007 subjects were sampled from the workers of Kailuan Coal Mine and surveyed with questionnaire Thorough physical examination was performed for all subjects Polymerase chain reaction (PCR) and electrophoresis were used to examine the insertion/deletion polymorphism of ACE gene PCR combined with restriction enzyme digestion, electrophoresis were used to detect the genotypes of AT1R gene Data were analyzed with logistic regression model and other statistical methods Results There was no statistical significance between essential hypertension and polymorphism of ACE gene or AT1R gene except DD+AC combined genotype Age, sex, snoring, body mass index, family history of hypertension, heart rate and hyperlipidemia were risk factors associated with essential hypertension after logistic regression analysis Polymorphism of the genes failed to enter the model Conclusions In the presence of other risk factors, risk for essential hypertension increased in the population with DD+AC combined genotype of ACE and AT1R genes However, the effect of single gene might be modest Environmental factors might be more important than genetic traits in the pathogenesis of essential hypertension
